5TH Grade End of Year Party

Dear Fifth Grade Parents,

It’s time for us to celebrate the end of Elementary School and the start of Junior High! We hope your fifth grader can join us on Wednesday, May 20, 2015 from 5:00 pm to 8:00 pm at Main Event Entertainment, 24401 Katy Freeway, Katy, TX 77494.

The cost of the party will be $25 per child. This will cover the cost of unlimited bowling, unlimited gravity ropes and unlimited laser tag. In addition, each child will be given a $10 video game card, 2 slices of pizza and unlimited soft drink refills.

There are several attached documents that will be required for every child.

Please keep all “End of Year Party” Forms stapled together.

• RSVP/Permission Form (please return even if not attending)
• Rylander PTA Release Form
• Car pool Consent Form(if your child is being picked up by an adult other than a parent.)

Please return your $25 payment together with the required forms to your child’s teacher no later thanFriday, April 17, 2015. When writing checks or money orders please make them out to “RRE PTA”. We are hopeful that every Rylander 5th grader will be able to attend this celebration! However, if we do not have payment and all required forms on file by April 17, your child may not be able to participate.NO EXCEPTIONS!

We will need adults to help chaperone at Main Event the night of the party. If you are interested in helping, please fill out the attached Volunteer Form and return to your child’s teacher along with the other forms.

RSVP and Permission Form

My child, ______, has permission to attend the 5th Grade End of Year Party on Wednesday, May 20, 2015 from 5-8 pm, at Main Event Entertainment, 24401 Katy Freeway, Katy,TX 77494.

I understand that the cost of the party is $25 and can only be paid with cash, check or money order. All checks should be made payable to “RRE PTA”.

I understand this signed permission form, as well as the signed release form, must be on file with the 5th Grade Party Event Coordinator before my child will be permitted to attend the party at Main Event Entertainment.

I understand that if I am unable to drop off and/or pick up my child myself, I must return the signed car pool consent form. This must be on file with the PTA 5th Grade Party Event Coordinator prior to the party, stating the adult responsible for my child’s check out. An adult MUST be with the child at drop off to check the child in to the party and receive check out instructions.

I understand that if there any behavior issues or irregular situations with my child, I will be phoned and requested to immediately pick up my child from the party. I understand that I must have a phone number where I can be reached on file with the 5th Grade Party Event Coordinator.

Volunteer Form

We will be in need of parent volunteers to help chaperone during the event and assist with student check-in, wristband disbursement, and dismissal. If you are interested in volunteering anytime from 4:30-8:30 on the day of the event, please fill out the bottom portion of this form and return along with your child’s other forms for the party.
