PDSA Worksheet for Testing Change

Cycle 1

Objective: Increase the number of dental sealants to where the proportion of patients with or receiving sealants is 50% of 6-8 year-olds and 75% of 11-12 year olds.

Every goal will require multiple smaller tests of change

Describe your first (or next) test of change: / Person responsible / When to be done / Where to be done
Collect baseline data / 8/17/09

Plan

List the tasks needed to set up this test of change

/ Person responsible / When to be done / Where to be done
Complete a DDS searchof children ages 6-8 and 11-12 who had at least one sealant between 8/17/08 and 8/17/09 / 8/17/09
Predict what will happen when the test is carried out / Measures to determine if prediction succeeds
Less than 25% of active user children in each age group will have at least one dental sealant / 1351 + 0007 + IH73/0000 for each age group.

DoDescribe what actually happened when you ran the test

38.86% of children ages 6-8 had at least one dental sealant between 8/17/08 and 8/17/09(82/211)

45.71% of children ages 11-12 had at least one dental sealant between 8/17/08 and 8/17/09(64/140)

StudyDescribe the measured results and how they compared to the predictions

The results of the measurement showed that a higher proportion of children ages 6-8 and ages 11-12 had at least one dental sealant between 8/17/08 and 8/17/09 than predicted.

ActDescribe what modifications to the plan will be made for the next cycle from what you learned

The purpose of this plan was to gather baseline data. No modifications are planned.

Cycle 2

Objective: Increase the number of dental sealants to where the proportion of patients with or receiving sealants is 50% of 6-8 year-olds and 75% of 11-12 year olds.

Every goal will require multiple smaller tests of change

Describe your first (or next) test of change: / Person responsible / When to be done / Where to be done
Collect baseline data / 8/17/09

Plan

List the tasks needed to set up this test of change

/ Person responsible / When to be done / Where to be done
Complete a DDS search of children ages 6-8 and 11-12 who received a dental sealant at the dental examination between 8/17/08 and 8/17/09 / 8/17/09
Predict what will happen when the test is carried out / Measures to determine if prediction succeeds
Less than 20% of children between the ages of 6-8 and between the ages of 11-12 will have received a dental sealant at the examination visit between 8/17/08 and 8/17/09 / 0150 followed by 1351 on the same visit/0150 between 08/17/08 and 8/17/09

DoDescribe what actually happened when you ran the test

No children between the ages of 6-8 (0/150)and between the ages of 11-12 (0/80)received a dental examination on the same day as the dental examination.

StudyDescribe the measured results and how they compared to the predictions

The results of the measurement are far less than expected, especially due to the fact that dental sealants are a major component of the Preventive Phase of the Ahfachkee School Linked Dental Program. Several requests have been made to staff at Quarterly Program Meetings to provide dental sealants at the examination appointment.

ActDescribe what modifications to the plan will be made for the next cycle from what you learned

Speak with Dr. Ricks to determine if correct formula of measurement was utilized. If not, the cycle will be repeated using the new measurement formula. If the correct formula was used, no modifications will be made to this plan as it’s purpose was to gather baseline data.

Cycle 3

Objective: Increase the number of dental sealants to where the proportion of patients with or receiving sealants is 50% of 6-8 year-olds and 75% of 11-12 year olds.

Every goal will require multiple smaller tests of change

Describe your first (or next) test of change: / Person responsible / When to be done / Where to be done
Re-Collect baseline data / 8/17/09

23.17% of children ages 6-8 received a dental sealant at the time of the exam between 8/17/08 and 8/17/09(35/151)

33.33% of children ages 11-12 received a dental sealant at the time of the exam between 8/17/08 and 8/17/09(64/140)

StudyDescribe the measured results and how they compared to the predictions

The results of the study were higher than predicted for both age groups. However, during the same time period there were 338 children between the ages of 6-8 and 209 children between the ages of 11-12 who had a visit to the Health Department. This means that 55.32% of eligible 6-8 year old users (338 – 151 = 187/338) and 33.01% of 11-12 year olds (209-140 = 69/209) did not have a dental examination between 08/17/08 and 08/17/09.

ActDescribe what modifications to the plan will be made for the next cycle from what you learned

Determine how much time is scheduled for dental examinations for children between the ages of 6-8 and 11-12 and determine what procedures are done during the examination.

Cycle 4

Objective: Increase the number of dental sealants to where the proportion of patients with or receiving sealants is 50% of 6-8 year-olds and 75% of 11-12 year olds.

Every goal will require multiple smaller tests of change

Describe your first (or next) test of change: / Person responsible / When to be done / Where to be done
Determine how much time is scheduled for an examination for 6-8 year olds and 11-12 year olds at site and determine what procedures are provided. / By 8/28/09

Plan

List the tasks needed to set up this test of change

/ Person responsible / When to be done / Where to be done
Complete a DDS search of children ages 6-8 and 11-12 who had at least one sealant between 8/17/08 and 8/17/09 / By 8/28/09
Predict what will happen when the test is carried out / Measures to determine if prediction succeeds
Dental examinations for children between the ages of 6-8 and 11-12 will be scheduled for one hour; sealants will not be included at the time of the exam. / Questions:
How much time is scheduled for a dental examination for children between the ages of 6-8 and 11-12?
What procedures are done at the time of the examination?
Who provides each service?
