The document is to be used by the Bidder to specify proposed rates for Air Conditioning, Heating and Ventilation Testing, Adjusting and Balancing – Requirements Contract, as outlined in this IFB.

The pricing table format below shall remain firm-fixed for one year (date of contract award begins the one year). Pricing shall include the cost for labor and any/all associated equipment needed to perform the services that may be required. Columns should not be altered; however, additional rows to capture specific detail may be added. Supplementary documentation may also be provided.

It is MANDATORY that you provide a Hourly Price for the services listed . Please enter any additional Hourly Price’s for services your company can provide that would enhance the Scope of work listed within this solicitation.
The bid prices shall include and cover all duties, handling and transportation, set up, supervision, charges and all charges incidental to the requested work Excluding Sales Tax or Use Tax. Offer shall agree to all MIHS Contract Terms and Conditions without exception.

Do not add travel time as an additional charge. Billing hours will begin when workers arrive at the jobsite and end when workers leave. MIHS will not pay for Lunch or breaks.

Trip Charges/Fuel Charges: No trip or Fuel charges are authorized under any resultant contract.

By submitting this Bid, Vendor acknowledges he/she has read and understands the entire Invitation For Bid and agrees to comply with its terms and conditions. The Vendor also agrees to fulfill the offer made in their Bid through any subsequently awarded Contract.

MIHS has the right to award contracts (if any) based upon the best interest of MIHS, no guarantee’s as to any type of award is made.
