We are committed to giving our patients high standards of care. We aim to work together with our patients and other members of the healthcare team to improve the health of our patients.

All patients registering with the practice are invited to attend for a new patient medical check. This will help us learn about you and improve the service which we can provide.

VIOLENCE AND AGGRESSION

The practice does everything possible to protect staff, patients and visitors from unacceptable behaviour.

All staff working in the Medical Centre have the right to carry out their duties without threat of violence or intimidation.

Appropriate action will be taken by the practice which may involve removal from the practice list and/or involvement of the police. This includes verbal abuse on the telephone.

We promise to treat all our patients with respect and courtesy and expect to be treated in the same way.

NON NHS SERVICES

Some services provided are not included in the NHS General Medical Services and these will carry a fee. Such services include certain vaccinations, certificates and medicals. You will be informed in advance if the service you request will carry a fee.

All charges will be in line with nationally agreed guidelines.

PRACTICE AREA

The practice covers the villages of Lesmahagow, Blackwood, Kirkmuirhill, Auchenheath, Boghead, Brocketsbrae, Dillarburn and Hawksland. Patients who live in the practice area may register with our Practice.

If you move out with the practice area you will be asked to arrange to register with a practice local to you.

SUGGESTIONS AND COMPLAINTS

We are always happy to hear suggestions patients may have to improve the services we provide. We do try to give the best possible service but there may be times when you feel this is not happening. If you have a complaint about any aspect of the service you have received, please let us know. We operate a practice complaints procedure in line with national NHS guidelines, copies of the procedure are available from reception staff and our Practice Manager, Fiona Clarkson, will be happy to speak to you on any matter of concern.

CONFIDENTIALITY

The practice complies with Data Protection and Access to Medical Records legislation. Identifiable information about you will be shared with others in the following circumstances.

·  To provide further medical treatment for you e.g. from District Nurses and hospital services.

·  To help you get other services e.g. from the Social Work Department. This requires your consent.

·  When we have a duty to others e.g. in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g. Diabetic Care.

If you do not wish anonymised information about you used in such a way, please let us know.

Reception and administration staff requires access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.

HOW TO SEE A DOCTOR OR NURSE

You can make an appointment with any doctor in the practice. We have surgeries starting from 8.10am until early evening. For certain examinations it will be necessary to arrange an appointment with both the nurse and doctor together. If you anticipate this may be necessary please let the receptionist know.

TELEPHONING THE PRACTICE

Appointments can be booked by phoning the surgery. Please try to avoid phoning first thing in the morning for routine appointments as the phone line is always busier then. Results will only be given over the telephone between 2pm and 4pm.

If you have an appointment which no longer require or are unable to keep please inform us as soon as possible.

HOME VISITS

If you are unable to attend the Medical Centre because of health problems we can visit you are home. A doctor will discuss your problem with you to ensure a home visit is necessary. Home visits are a valuable service and vital to the housebound and terminally ill. Please do not misuse this service be requesting home visits inappropriately. If your condition is urgent but you can come to the Medical Centre we may be able to see you more quickly there. If you require a home visit we encourage you to phone between 8.00am and 9.00am so that the visits for the day can be planned efficiently.

REPEAT PRESCRIPTIONS

If you are prescribed long term medication you will be issued with a repeat prescription request slip. This is printed on a tear off sheet next to your prescription. A few days before you need a further supply of your medication complete the slip and take it or post it to the surgery. We will organise your prescription in 2 working days of receipt. If you have run out of your medication and need a prescription immediately please inform the receptionist. If you wish your prescription to be taken directly to the chemist please allow 2 full working days. If you stop taking any medication please help us to keep your records up to date by scoring it off the list.

Repeat prescriptions are not taken over the phone as this would result in the phone being constantly engaged. Repeat prescriptions can be ordered on the practice website: glebemedicalpractice.co.uk

SERVICES AVAILABLE

As well as providing general medical services the practice also provides maternity services, cervical smears and child health surveillance. If you are travelling abroad and require vaccinations please ensure you make an appointment with the Practice Nurse AT LEAST 6 weeks before travel. Please ask at reception for details.

HELP US TO HELP YOU

Please use the touch-screen in the reception area to let us know you have arrived for your appointment. If you require assistance please ask the receptionist. The doctor or nurse will be unable to call you if your arrival has not been recorded on the appointment system. The surgery has ramps to the entry and exit doors to facilitate wheelchair use. If you require any assistance please inform reception and we will be delighted to help. It is our job to give you treatment and advice. In the interest of your health it is important for you to understand all the information. Please ask us if you are unsure of anything. Please inform us of any change of address or telephone number you make in order to keep your records up to date.

COMMUNITY NURSING STAFF

The HEALTH VISITORS are general Nurses with further training in health care for all ages, but especially families with young children. She offers a service at home or at the Practice.

The DISTRICT NURSES work with the Doctor and perform nursing procedures in your home for anyone not able to come to the surgery. The District Nurses can be contacted Monday to Friday 8.45am to 4.45pm on 01555 892899 or the Health Visitors on 01555 895985. Messages can be left on the Answering Machine when Health Visitors and District Nurses are visiting patients.
