Social prescribing –

Social prescribing, sometimes referred to as community referral, is a means of enabling GPs, nurses and other primary care professionals to refer people to a range of local, non-clinical services. Groups have been set up in different surgeries and areas for things such a painting group for children suffering from asthma, or, a knitting group for adults with diabetes. A community group for patients who can find support in one another.

The surgery would be happy to support the PPG if they had a member that wanted to champion a group to support a particular patient demographic. It is possible to obtain funding for such things.

Prescribing incentive scheme –

Prescribers are encouraged to prescribe medicines which are both effective clinically and financially. An incentive is paid for the use of less broad spectrum antibiotic (Eg. Co-Amoxiclav) because of the well documented increasing resistance of antibiotics in bacterial infections.

The practice must use the funds raised for agreed resources for the benefit of patients, such as a new blood pressure machine or the new chairs in the waiting room. This year it is planned to buy a new built for propose phlebotomy chair, and a new waiting room information system will be purchased from the funds also. Being part of the scheme also opens up funded training opportunities for staff. Jane Corbet, MK CCG Medicines management, has monthly dashboards of targets for the surgeries of Milton Keynes.

2. Report from the Practice

The surgery continues to work on the appointment system and have begun trialling asignposting system. This involves, initially, two key members of the reception team signposting patients from 8am – 9am to the most relevant clinician or outcome. They do this by asking patients some questions regarding their needs. This is very much in its infancy but will include regular signposting meetings with the core members of the team involved with review and measurement of the changes.

The aim is that every patient contact ends in resolution and that patient’s see the most relevant clinician to their needs whether that is an Advanced Nurse Practitioner, Health Care Assistant, Nurse or GP and to empower the reception team with their skills.

Dr Nancy Murray and Emma continue to work on a script for the new opening telephone message which will be read by Dr Murray.

Dr S Dahanayake has unfortunately left the practice after a trial period. The surgery is actively advertising for a salaried GP.

A ‘children’s corner’ is still on the agenda for the waiting room.

Flu clinics start 23rd September 2017 with midweek and Saturday clinics available. Members of the reception team will be running a Macmillan coffee morning on the 30th September and, the PPG have volunteered to run a further Macmillan morning on the 7th of October. Please contact PPG Chair, Janet Smith for further details.

Pam Handly (HCA) has been trained to administer flu vaccines alongside our existing nursing team.

The surgery has purchased some home BP machines that can be sold to patients at cost price (£20) for anyone that requires one.

The attending group unanimously nominated Janet Smith and, as enough members were present for quorum, Janet accepted. Walnut Tree Health Centre welcomes this decision.
