Purpose of Trip:TO INVESTIGATE SPACE, PLANETARIUM PROGRAM, “OUR PLACE IN SPACE,” AND PARTICIPATE IN HANDS-ON ACTIVITIES THROUGHOUT THE SCIENCE CENTER.
Chaperones:Chaperones must pay in advance by this DUE DATE; No one may pay at the door as part of our group—Also, the ASC Membership passes or discounts cannot be used for this group fieldtrip per policy of ASC.

Additional Facts Necessary For Parents to be fully Informed:
Field Trip will be cancelled if we do not collect enough funds to cover the cost of the trip.
** Since the adult chaperones’ primary responsibility will be in assisting groups of students, no younger siblings are permitted with chaperones on this fieldtrip.

I fully understand and agree that because of the risk associated with any field trip my child is expected to 1) act and behave appropriately at all times, 2) abide by all school and safety rules, including the fact that cell phones are not to be used or accessed during normal school hours (including field trips), and 3) follow directives and instructions of any teacher, administrator, chaperone or any other adult associated with the field trip. Furthermore, I understand that my child may be subject to appropriate disciplinary action for his/her failure to do so. All care and supervision will be exerted to prevent any accident from occurring. However, in case of an accident your signature indicates that you will not hold the school or sponsors responsible and it gives your child permission to participate in the field trip.
**Chaperones: The purpose of being a chaperone is to help supervise the students and to assist the teacher(s). All students are expected to remain with the class at all times. Parents/chaperones are encouraged to stress the importance of attendance by allowing the student to remain in school until their regular dismissal time.
