Practice population profile
Please provide information on the practice profile:
Current practice list size is 3122 (As on 29.03.2014)
  • We have 1667 males and 1455 females.
  • Age 0-4 : we have 184patients
  • Under 65s (over 4yrs) : we have 2601 patients
  • Age 65-74: we have 198 patients
  • Over 74 : we have 139 patients
The practice has a mixed ethnic profile.
The percentages of the ethnic origins of total practice population would be approximately as below.
  • White/ Europeans: 4%
  • Asian / British Asians: 81%
  • Black / mixed : 13%
  • Other : 2%

PRG profile
Please demonstrate how the practice has ensured that the PRG is representative by providing information on the PRG profile
In forming our patient participation group we have tried to ensure the group is representative of our practice profile. We displayed a poster in our practice to invite patients to join the group. We started by inviting those patients who had shown an interest.
At present the group consists of 20 members,0 virtual memberbut we hope by advertising on the website and by putting posters up in the surgery that this number will grow.
The practice has high population were English is not their first language, the invitation poster to join the PRG in different language is displayed and also we have agreed that the group of interested patients who could not come to meetings will be contacted by telephone or email for their views.
The practice PRG profile is as below:

Differences between the practice population and members of the PRG
Please describe variations between the group and what efforts the practice has made to reach any groups not represented.
The practice actively encouraged all patients to participate but we only received a few expressions of interest. We will continue to encourage patients to join the group.
Practice will also try to engage underrepresented group my displaying invitation poster in different languages.

Component 2: Agree with the PRG which issues are the priority and include

these in a local practice survey.

Please describe how the priorities were set
The practice met with the PRG group on 25th March 2014 and developed terms of reference. Areas of concern were discussed and where it was felt improvement was needed it was agreed with the PRG group. The patient survey questions were developed from the discussion at this meeting. Outcomes would be fed back to the PRG and virtual group. These points will also be discussed in the practice meeting for review and discussion.

Areas of concerns were identified by the PRG group. These areas will be used to develop the practice patient survey.
Please describe how the survey was conducted?
At present we are distributing the survey forms to all the patients attending the surgery and we will continue until we reach the minimum patient survey requirement

Opening times

Monday: 8.30 to 18.30
Tuesday: 8.30 to 18.30
Wednesday: 8.30 to 18.30
Thursday: 8.30 to 13.30 (closed in the afternoon)
Friday: 8.30 to 18.30
The practice is closed on Saturdays, Sundays and on public holidays.
