Role Profile

Senior Job Coach– Post No. TM348

Overall Contribution

The role holder is responsible for:-

  • Providing line management to the Job Coaches.
  • Acting as a point of contact for stakeholders including Hampshire County Council and employers.
  • Role modelling sound professional practice.
  • Ensuring service users are supported in all work placements or activities identified as part of the pathway to work.
  • Enabling service users develop a plan for timely exit from the supported employment service after three months.

Key Result Areas

1.Provide leadership, support, advice and professional supervision to the Job Coaches to achieve best practice.

2.Plan, implement and evaluate programmes that support service users to maintain as independent as life as possible, ensuring all needs and risks are identified and addressed in an individually tailored plan of support. Where appropriate, and if required, assess and implement relevant Risk Assessments in the workplace.

3.To provide support for service usersin their workplace.

4.Act as a key worker for that service user for the specific period.

5.Work with employers to provide “reasonable adjustments” for a service user referral.

6.Interact and participate with service users in the continued development of self-help skills to ensure that they reach their full potential for independence.

7.To ensure that regular feedback and monitoring of service users’ progress is communicated to all parties, particularly the JOC.

6.To act as the lead with the employer when there are difficulties within the placement.

7. To support service users in the use of Assistive Technology included in their support plan.

8. Manage an effective exit strategy of support for service users.

9. Ensure that effective records are kept of all service users.

10. To provide service users with job advice and guidance and update their action

plans.

11. Have an awareness of the wider social care sector generally and learning

disability specifically.

12. Have an understanding of equalities issues and how to promote people’s

equality, diversity and rights.

13. Demonstrate an ability to work effectively in a partnership environment, eg

working co-operatively with other agencies including colleagues from our Adult

Services Integrated Community Teams.

14. Understand data protection/confidentiality and the need for accurate and timely

recording.

15. Be organised – able to manage time and conflicting demands.

16. Ensure that alternative sources of funding are accessed that will offer any

continued support after the initial three months, eg with Direct Payment and Job

Centre Plus benefits.

17. Identify and overcome any barriers to the success of the work placement or job.

18. To support employer staff training to ensure that the service user settles in.

19. Meet the travel requirements of the role.
