Instructions:

1. The sponsor is required to notify the local health department of:
2. The planned summer meal service sites before site operations begin.
3. New sites, closed sites, and meal service changes (dates, days, times, meals) as they occur throughout the summer.
4. The sponsor is required to submit a copy of the initial health department notification letter with the Sponsor Application Packet.
