MAIN PURPOSE
The post holder will contribute to tackling the consistently high levels of obesity and inactivity within the London Borough of Richmond upon Thames, through delivery of a range of lifestyle programmes.
SUMMARY OF RESPONSIBILITIES AND DUTIES
  • Plan, coordinate and implement opportunities to promote the Richmond Live Well Stay Well Service to members of the public and other key stakeholders including GPs, Dietitians, Wellbeing and Mental Health groups, practice nurses, school staff,school health nurses, voluntary agencies.
  • Plan and implement a range of programmes in both groups and one to ones settings. This will include delivering facilitated discussions around evidence based nutrition and physical activity advice and supporting participants to adopt behaviour change techniques.
  • To support delivery of NHS Health Checks and Walking Away from Diabetes within community settings across Richmond.
  • Be accountable for the monitoring and achievement of contractual key performance indicators (KPIs) and other service related targets
  • Work alongside the Lifestyle Hub to ensure administrative tasks are undertaken according to service protocol.
  • Keep accurate records and written reports, and ensure patient contact data is recorded in accordance with Mytime Health and NHS Trust policies, to ensure compliance with Information Governance and Data Protection in relation to client and patient information.
  • Respond to any relevant clinical/ staffing issues / incidences to manage mitigate risk and maintain best practice.
  • To manage and organise own time and activities to ensure individuals are provided with a service and supportive environment.
  • To keep abreast of changes within the food, nutrition, health and fitness industry and ensure knowledge is up to date through the reading of literature, attending seminars and continuing professional development.
  • To be an ambassador for Mytime Active and contribute to the growth of the Health division by working collaboratively with Managers / colleagues to develop and have responsibility for an expanding portfolio of services and products.
  • To demonstrate an understanding of the principles of Equal opportunity and demonstrate an appreciation of the ways in which the needs of a diverse population can be served
  • There will be a requirement for some evening and weekend working.

SKILLS & ABILITIES
  1. Well developed verbal and written communication skills in order to effectively communicate with a wide range of individuals including participants, colleagues and external partners.
  2. Ability to organise, monitor and check work to ensure no element of a task is left unfinished.
  3. Proven ability to work on your own initiative and as part of an effective team to achieve the Health Division core objectives and contractual KPIs.
  4. Have an awareness of patientneeds and a commitment to continuous service improvement so that all patients receive a highquality service.
  5. Able to respect confidentiality even in difficult situations.
  6. Able to be supportive and encouraging to people in difficult situations without being judgemental.

KNOWLEDGE
  • Knowledge of Nutrition and Physical Activity Guidelines and understanding of value of exercise and healthy eating in relation to health improvement.
  • Detailed Knowledge and understanding of behaviour change techniques, including motivational interviewing, in relation to supporting people to make positive lifestyle changes.
  • Knowledge and understanding of the Patient Confidentiality Act and the Data Protection Act.
  • Knowledge and understanding of the local community.
  • Knowledge of local services and how to help others to use them.
  • Ability to use generic software packages e.g. Microsoft Word, Excel, PowerPoint.
  • Ability to develop skilled use of Mytime Actives bespoke database programme

  • Experience of working in a health/ exercise setting
