1.1 Preventive maintenance i.e daily, weekly, fortnightly, monthly, quarterly & checks of all the abve equipments

1.2 Shutdown maintenance i.e half yearly and yearly checks of all the equipments.

1.3 Breakdown maintenance of all the equipment.

1.4 Filling the log book/ job card on shift basis, filling the checklists after carrying out reventive/ shutdown maintenance & updating the preventive/ shutdown/ breakdown maintenance entries & parts consumed in the computer present in the work area.

1.5 Upkeeping & maintenance proper cleanliness the work area/ stores.

1.6 The work is to be done in all working days (Monday to Saturday) in GeneralShift from 08.00 to 16.45 hrs.

1.7 However your personnel are to work after the completion of the above- mentioned shifts for completing the job. If your personnel are absent during working hours without any valid reason, the penalty of Rs 200/ day/ person will be deducted form your payment.

1.8 On Sundays or national holidays work is to be done only if an emergent situation arises.

1.9 PF & ESI of your manpower is entirely your responsibility.

1.10 Safety of all your manpower is entirely your responsibility. Uniform (Including safety shoes, safety gadgets) will be provided by you. You must ensure compliance with all safety and health regulation in force during the period of contract. You will have to ensure that only those persons will be employed for carrying out the job those are adequately trained in safety and technical aspects as required for the job.

1.11 All the tools shall be provided by BHEL.

1.12 All the consumable materials and spares used in maintenance of fork lifters shall be arranged by BHEL.

1.13 As and when overhauling of engine and transmission system of fork lifters required same shall be carried out by BHEL against separate approvals. Scope for the above work shall recommended by vender after detailed inspections.

1.14 Welding, brazing & gas cutting jobs to be carried out as & when required.

1.15 Drilling, tapping, Recoil fitment & any other fitting job to be carried out as & when required.

1.16 Follow- Up with the in house tool shop for any machining work.

2.0CANCELLATION OF CONTRACT IN FULL OR PART.

Contract shall be cancel in full or part if the contractor:

2.1At any times makes defaults in proceeding with the work with due negligence and continuous to do so after notice in writing or 7 days from the Sr.DGM (WE&S) BHEL, Jhansi.

2.2Commits defaults in complying with any if the terms and conditions of contract.

3.0PERIOD OF CONTRACT

The period contract will be for one year.

4.0DEPLOYMENT OF MANPOWER

The contractor has to provide sufficient manpower of experienced mechanic/electrician and suitable helpers to assist the total work during contract period.

5.0 TERMS OF PAYMENTS

5.1Payment will be made every month on submission of work done during 4the periodduly verified by erection in charge.

5.2Income tax will be deducted from the each bill. This will be as per guideline of govt. of India. At present 2% income tax will be deducted.

5.3Service tax will be paid, this will be as per guideline of govt. of India. At present service tax is 12.36% contractor should be submitted Challan copy of service tax deposit.

6.0GENERAL TERMS & CONDITIONS

6.1Contract shall keep all staff orderly in proper uniform and arrange to provide them the work of repair/ manufacture of spares in their idle times.

6.2No foreign/ used/ unused/ scrapped/ broken parts shall be left on area.

5.3All used/ worn-out/scrapped/ replaced spares/ parts shall remain the property of BHEL, Jhansi. These items shall be return to WE&S store.

6.4BHEL, Jhansi shall not be liable for any compensation what so ever in the event of accident/ injury to the persons employed by the contractor however, first aid facility shall be rendered free of cost.

6.5Utmost care should be taken by the contractor to repair the items proper way & with quality workman ship so as to satisfy requirements as per factory act. 1948 section 19 & U.P. Factories Rules 1950 Rule 55-A & subsequent amendments enforced from time to time.

6.6As our company being accorded with ISO 900A, ISO 14001 and OHSAS 18001, contractor are required to carry out work confirming to our quality standards along with necessary documentation / paper work.

6.7In the event of requirement of material / spares etc. contractor will approach maintenance in chare along with required paper work, which will authorize/ counter sign the same for withdrawal. Contractor will make own arrangement to collect the material / spares from is initial point of supply of store.

QUALIFYING REQUIREMENTS

1.Should have independent ESI Code number.

2.Should have PAN/TAN number.

3.Should have Service Tax number (PAN based).

4.Self certificate that he is not blacklisted/banned/delisted on the date of tender.

5.Should have VAT/TIN No. if material is supplied/consumed.

6.Declaration that he is not blacklisted/under hold from BHEL Jhansi or banned by any unit/region/office of BHEL.

7.Declaration that he should not be guilty by a Court of Law in India for any offence involving fraud, dishonesty and moral turpitude.

  1. Similar completed work in last 7 years ending 31st Aug-12 should be any one of the following:
  2. Three completed work contract for the work of maintenance of Fork Liftersin last 7 years ending 31stAug-12 ,each costing not less than the amount equal to Rs 04.31 Lakh.

OR

  1. Two completed work contract for the work of maintenance of Fork Lifters in last 7 years ending 31st Aug-12 ,each costing not less than the amount equal to Rs 05.39 Lakh.

OR

  1. One completed work contract for the work of maintenance of Fork Lifters in last 7 years ending 31st Aug-12 ,costing not less than the amount equal to Rs 08.63 Lakh.

9. Average annual financial turnover during last three years, ending 31st March. 2012 not less thanRs 3.24 Lakhs.

Documentary proof in support of above eligibility criteria shall be submitted along with Techno Commercial Bid at Annexure I

INSTRUCTIONS TO TENDERERS

-Above tender is in two Bid System (Techno Commercial Bid & Price/ Rate Bid)

1.The tender is to be processed in two parts viz. 1) Techno Commercial bid 2) and price/rate bid. The Technical Bid Application (Annexure - I) and its enclosures (along with the Contractor’s Obligations, General Terms & Conditions and copies of qualifying requirements ) must be submitted in one sealed envelope superscribed as “Tender for “annual maintenance CONTRACT FOR FORK LIFTERS IN BHEL-JHANSI,TECHNOCOMMERCIALbid”.EMD/Cost of Tender Document or Documents required as per qualifying requirements must be enclosed. The second envelope duly sealed should contain the price bid (Annexure-J) only super scribed as “( Name of Work)“annual maintenance CONTRACT FOR FORK LIFTERS IN BHEL-JHANSI”,. PRICE bid”. Any other enclosures, which the tenderer wishes to submit, must be enclosed with the Technical Bid only. The price bid envelope should contain the rates only. Offers not in line with the above procedure or quoted in any other format is liable to be rejected/by passed.

2.Both the technical bid and price bid sealed envelopes must be again sent in a single envelope duly sealed and super-scribed as “TENDER FOR TENDER NO: weX/TE/2/07 Due date of opening 28.09.2012 The same should be dropped in the tender box kept in the CISF gate of Administrative Building, BHEL, Khailar, Jhansi, within the specified date and time by the representative of the tenderer.

3.All tenderers can witness the opening of the Technical Bid. After evaluation of the Technical Bid which may involve visit to the tenderer’s/ clients place by authorised officials, price bids of only those tenderers who are technically found suitable will be opened on a subsequent date which will be informed to the concerned whose technical bid has been accepted, in advance for witnessing the Price Bid opening.

4.All entries in the tender document should be in one ink. Corrections, over writing, cuttings, etc. will not be permitted. All the columns in the tender form should be filled without leaving blank in any page of the tender and all the pages must be duly signed & stamped by the tenderer before submission.

5.The price / rate should be quoted in figures as well as in words. Price quoted in Price Bid shall be inclusive of all taxes & duties, all statutory requirements/liabilities like PF/ESI/Bonus/Min. wages etc. However service Tax shall be payable extra as applicable.

6.The rate quoted will be valid for the period of contract.

7.The contract will commence on the date as applicable against the contract and shall be valid as per the validity period of the contract. The contract can be extended further on mutually agreed terms and conditions.

TENDER EVALUATION CRITERIA

The evaluation of the Tender will be carried out as follows:

  1. BHEL officials may visit the tenderer’s office & their client’s place to evaluate the capability and assess the performance.

Capability of the Contractor will be assessed on the basis of employees including supervisors on their rolls, machinery, equipments in its possession, previous track record, experience in other organizations, etc.

  1. Successful techno commercial qualified tenderers after the above process only will be invited for opening of the rate / price bid.
  1. The contractor shall be awarded the work on over all L-1 basis if otherwise not specified in the tender.

CONTRACTOR’SOBLIGATIONS

A)CONTRACTUAL

Towards selection, control and supervision of employees

a)Contractor shall decide the number of employees to be deployed for execution of the work awarded to him and he or his authorized representative will be solely entitled to instruct such workers about the manner of carrying out the work as per the prescribed specifications and quality plan.

b) Contractor shall supervise the work allotted to him and to be carried out by his employees.

c) Contractor to ensure that the employees deployed in the premises of BHEL are physically and mentally fit and do not have any criminal record. Such employees should possess requisite skill, proficiency, qualification, experience etc.

d) Contractor to maintain appropriate records of his employees deployed to carry out the job(s).

e) Contractor shall ensure Group Insurance Policy for his workers.The coverage shall be of minimum Rs 3 lakhs per individual.

f) Contractor will ensure that the job is executed through his employees on his rolls and under no circumstances the contractor will deploy any casual employee to carry out the job nor shall sub-contract the job without prior written permission.

g)Contractor will keep watch on his employees and he will be liable for any pilferage / loss to BHEL due to acts of omission and commission by his employees. Similarly, liability for any compensation to outsiders on account of any act of omission and commission by the employees deployed by the contractor shall lie exclusively with him.

h)The contractor has to provide to his workers uniform ( lower dark blue & upper light blue) along withbadges bearing logo of his establishment, which shall be affixed by the contract labour on the left side of his pocket.
