Indicate/explain if you are entering as an automatic berth resulting from (1) a regional or world first place finisher in the 2010 IFSS World Cup, (2) a gold medalist in the 2009 Daaquam World Championship, or (3) a participant in the 2010 Training Camp in Sweden. You are still required to submit application and entry fees to MUSA. If any of these this apply to you, you may omit sections on Mushing Record and Outcomes. World Cup 2010 Results can be obtained from IFSS Secretary General Sally Bair () or online at Cup>Results>2009-2010.

Classes Entered/Entry Fees:With your application, please include a check, made out to Mushing USA, for your total entry fees. It will be returned if you are not selected. Check below the classes you wish to enter. Entry fees have been set by the host (Norway) and include transfer and currency conversion fees. Note: RNB = Registered Nordic Breed. A Nordic Relay will be offered, but team composition will be determined in Norway and payment made there (100 €/team).

Applicant’s Competition/Mushing Record

In order to make a decision for entry into a World Championship, MUSA needs to know your race record. Please indicate below your snow racing record for the last one or two years/seasons preceding the year you are applying for. Indicate the record for only the snow class and category in which you wish to compete representing the USA. Use additional sheet if necessary. Indicate any past IFSS World Championship (WCh) in which you competed in the class for which you are applying or any past IFSS World Cup (WCp) awards earned. Your first listed event must be the 2010 race that was IFSS accredited.

Medical Insurance:

You will need proof of medical insurance if your application is accepted. If travel is to a foreign country, you will need to check with your medical insurance company to make sure you are covered in a foreign country. Neither MUSA nor IFSS is liable for any injury incurred to you or your dogs either directly the result of competition or in the duration of your travel.

Medicine Prescribed by Physician or Veterinarian

If you or any of your dogs are taking any prescription medication, you will need proof of this from your physician or veterinarian if your application is accepted. Please be aware that you and/or any of your dogs may be selected for drug testing under the guidelines of the World Anti-Doping Agency (WADA) and the WADA Anti-Doping Code and the Anti-Doping Rules of IFSS (both human and canine). If the prescription is for you, you will need to submit a Therapeutic Use Exemption to IFSS. Contact IFSS Secretary General Sally Bair () for form, or download from the MUSA website ( Programs>Anti-Doping>Anti-Doping TUE Form).

Identification:

If you are traveling to a foreign country, you must have a valid United States passport. Mushing USA accepts applications from only US citizens or by petition. You may be required to provide verification of citizenship to MUSA with either a passport or birth certificate.

Deadline:

Deadline by which to submit applications to MUSA will be September 15, 2010.
