MoASBO-MUSIC SCHOLARSHIP GOLF TOURNAMENT

The Lodge of the Four Seasons- Cove and Ridge Golf Courses

The annual MoASBO/MUSIC Golf Tournament is scheduled for Tuesday, April 21, 2015 prior to the MoASBO Spring Conference. All revenue from the event will be divided evenly between the two organizations and will go to sponsor their respective student scholarship programs.

As a business associate we look forward to your participation in this event and trust you will find it to be an excellent opportunity to showcase your company as well as support these worthy endeavors.

REGISTRATION INFORMATION

Complete the registration form for the team you want to enter. You are allowed to sponsor more than one team. List the company representative that will be playing in the event. The registration cost per player is $60. This amount includes: golf and cart, two drink tickets on the course and the dinner at the awards presentation. The awards dinner will be held on the Patio at the Cove Golf Course. How many will attend? ______

If you do not register a team, we will attempt to place you with active MoASBO/MUSIC members; however, we are unable to guarantee all of your team members will be active members of our organizations.

There will be two flights(on each course) with cash payment of $75.00 for 1st place, $50.00 for 2nd place and $25 for 3rd place.

Mulligans can be purchased at your course for $20.00 per team, 2 mulligans per player.

SPONSORSHIPS

In addition to your registration for the tournament, there are also opportunities for additional sponsorships as well as door prizes. All associates who sponsor an activity or provide a door prize will be recognized at the awards dinner and in the MoASBO Spring Confernce Program and in the MUSIC Matters Newsletter.

Hole Sponsors-$200 This amount includes one player registration fee, 2 drink coupons

on the course, 1 dinner at the awards presentation and a sign with your company’s name

on the tee box at each course. The cost for additional company representatives is $60.00.

Longest Putt- 4 sponsorships, two on each course at $50 each.

Door Prizes-If you would like to donate a door prize to be given away at the Awards

Dinner please indicate that prize and bring it to the dinner.
