The section for self-identification assists us in demonstrating our efforts in recruiting a diverse faculty. Your assistance in providing this information assures VTCSOM remains compliant with our accrediting agencies.

___3.Completed Faculty Involvement Form(attached)

A faculty appointment requires participation in the fulfillment of the mission of VTCSOM through teaching, assessment/evaluation, curriculum development, administrative duties or other types of service. MOA certification may involve resident/fellow oversight or teaching activity assuming those residents/fellows interact with VTCSOM students.

The Family Educational Rights and Privacy Act (FERPA) of 1974 is a Federal law that protects the privacy of student education records. The law applies to all schools receiving funds under an applicable program of the U.S. Department of Medicine.
