Congratulations on your recent graduation and best wishes for your future plans! GEAR UP for SUCCESS is pleased to provide a College Readiness Tool (CRT) Kit for those planning to continue their education toward a college degree in the fall. The CRT Kits were ordered based on student responses to the College Plans Survey. Below you will find information about your eligibility for receiving a CRT Kit or the possibility to be placed on a Wait List to receive one as funds allow.

• A CRT Kit has been ordered for you if you completed a College Plans Survey card indicating you intend to attend college fall 2014AND listed at least one college to which you have been accepted AND submit the documentation below. If you cannot be present on Distribution Day, your CRT Kit will be left with the school contact person who will collect your documentation and distribute your kit. CRT Kits must be claimed within 10 days of the Distribution Day or they will be returned to GEAR UP for SUCCESS.
  

• You may have been added to the Wait List if you said you intend to attend college in the fallBUT did not list at least one college to which they had been accepted. Submit the documentation below on distribution day and you may either receive a CRT Kit that day OR receive notification to pick up a CRT Kit from your high school for pickup as long as funding allows.
  
• Students who did not have the opportunity to complete a College Plans Survey Card,may also be placed on the wait list by presenting on distribution day documentation of acceptance and/or enrollment for the fall 2014. While there is no guarantee the students will receive a CRT Kit, the intent is to purchase and sendkitsto the students’ high schools for pickup as long as funding allows.

To receive a CRT Kit or be placed on the Wait List, students must submit two things:

1. Memorandum of Understanding (provided by GEAR UP for SUCCESS) signed by the student.
2. EITHERProof of enrollment for fall 2014 in a college or university which offers associates or bachelor’s degrees.
   

OR

Proof of acceptance for fall 2014 AND a date for enrollment or orientation.

• Students who wish to be added to the Wait List must either present the signed Memorandum of Understanding and the Proof described above on Distribution Day or send by email, FAX, mail or in person no later than JUNE 15 to:

• Unfortunately, due to US Department of Education’s regulations, we can only provide the CRT Kits to students attending 2-year or 4-year colleges/universities (schools from which students graduate with associate’s or bachelor’s degrees). Purchase of the CRT Kits for students who plan to attend Technical/Trade schools from which they will receive certification is not allowable from grant funds.
