LEARNING AGREEMENT

This agreement must be filled out completely, signed, and returned to the Fulton Schools of Engineering Career Center before you start your Internship/Co-op. Clear definition of the employer’s expectations and the student’s measurable learning objectives will maximize the Internship/Co-op experience for both parties. Please type or write legibly.

Provide a job description which includes the nature of the position; your duties; specific projects or tasks; and the required skills. The site supervisor and student complete this section together.

Internship/Co-op Measurable Learning Objectives (MLOs)

What do you plan to learn from this experience that advances your understanding of coursework concepts? How does this internship/co-op contribute to your ability to enter a career with experience? List specific educational, professional and personal goals that are observable, measurable, and able to be evaluated by your faculty mentor and site supervisor. Please use the guidelines set forth in the “Instructions for Writing Measurable Learning Objectives” handout from the Fulton Schools of Engineering Career Center. Note: Your department may have standardized departmental learning objectives.

Internships/Co-ops for academic credit must have signed approval by anASU approved faculty mentor. Learning agreements must be submitted to theFulton Schools of Engineering Career Center before you register for internship/Co-op credit.

RESPONSIBILITIES UNDER THIS AGREEMENT

The student, employer, faculty mentor, and FSECC internship/co-op coordinator agree to assume the responsibilities outlined below for the duration of the student’s internship/co-op placement. NOTE: Employer, faculty mentor and internship/co-op coordinator signatures are required prior to the start of an internship/co-op carrying academic credit.

Employer Responsibilities (1st signature):

  • Provide a comprehensive job description to verify the position and duties meet internship/co-op criteria.
  • Sign an ASU Affiliation Agreement
  • Assist the student in formulating concise and clear measurable learning objectives for the internship/co-op.
  • Provide an orientation, any precautionary safety instructions, training and ongoing supervision for assigned duties.
  • Provide regular feedback to the student regarding performance.
  • Notify the Engineering Career Center in a timely manner if any problems arise during the internship/co-op placement.
  • Be accessible to internship/co-op coordinator for phone calls, emails, and/or site visits.
  • Approve monthly time reports verifying hours worked.
  • Complete a final written evaluation of the student’s performance (online evaluation forms will be provided by ECC).

Faculty Mentor Responsibilities (2nd signature):

  • Determine and discuss how the proposed internship/co-op is relevant to the student’s educational objectives and merits academic credit.
  • Review student’s measurable learning objectives for the internship/co-op.
  • Make recommendations on assignments, project scope and other areas as needed.
  • Keep in contact with the student during the semester to offer guidance and support.
  • Be accessible to internship/co-op coordinator for phone calls and/or emails.
  • Review submitted course materials and assignments.
  • Assign the final grade.

Student Responsibilities(3rdsignature):

  • Fulfill those tasks and responsibilities assigned by your site supervisor and outlined in this Learning Agreement.
  • Conduct yourself in an ethical and professional manner, acting as a positive representation of ASU as outlined in the ASU Student Handbook.
  • Complete a minimum of 45 hours for each 1credit hour received (e.g. 135 hours = 3 credit hours, etc.).
  • Have a medical insurance plan that will provide adequate coverage during the internship/co-op.
  • Notify ECC and your faculty mentor in a timely manner of any changes to this Learning Agreement or if any problems develop during the internship/co-op.
  • Complete the evaluation of your internship experience (online evaluation form will be provided by ECC).

ECC Internship/Co-op Coordinator Responsibilities (final signature):

  • Review the student’s eligibility for participating in an academic internship/co-op (e.g. GPA, prerequisites, etc.)
  • Serve as liaison for employer, faculty mentor, student, and dean’s office.
  • Review the Learning Agreement for completeness and accuracy of information.
  • Administer internship evaluation process for all parties.
  • Assist the student, the site supervisor, and the faculty mentor in resolving any issues that arise.
