Instructions for Filling Application

Application Document Checklist

The application should contain the following documents.

A) Forms to be Downloaded and Submitted

①□ (Form 1) Application Document Checklist (This form)

②□ (Form 2) Certificate of Health

③□ (Form 3) Certificate of Student Registration (Should be filled out by the home institution)

④□ (Form 4) UMAP Credit Transfer Scheme (UCTS) Study Plan

⑤□ (Form 5) Confirmation of Student Registration (Should be filled out by the home institution)

⑥□ A. Detailed Plan of Research (Special Research Student only/ Free format)*

*It is required to state the name of your prospective supervisor at HU who has agreed to supervise your research.

□ B. (Form 6) Statement of Purposeand Proposal for Internationalization Support (Both Special Auditing Students [Undergraduate or Graduate Students] and Special Research Student)

⑦□ (Form 7) Letter of Reference

⑧□ (Form 8) Certificate of Bank Account (Should be officially certified by the bank) **

⑨□ (Form 9) Statement of Financial Support (Letter stating the financial supporter's relationship to the applicant) **

**Applicants are required to submit the Certificate of Bank Account and Statement of Financial Support in order to prove financial status. These documents will be submitted to the Immigration Bureau by HU to apply for your “Certificate of Eligibility (CoE)” in July, 2018. The “financial supporter” needs to be consistent and identical with the person stated in your on-line application for “CoE” to be submitted in late May, 2018.

B) Other Documents to be Submitted

①□ 2 passport type photos (Applicant’s name and country name should be written on the back side.)
Your photos must be:
●30 mm wide ×40 mm long (1.2 inches wide ×1.6 inches long).
●clear, sharp and in focus. Photos may be in color.
●reflectyour current appearance (taken within the last 12 months).
●professionally printed on plain, high quality photographic paper (photos printed at home are not acceptable)

②□ Officially Certified Academic Record
□ Explanation of grading system in the Academic Record if academic record dose not list it
□ GPA (Grade PointAverage) if academic record does not list it

③□ A copy of TOEFL score or IELTS score (Non-native English speaker only) OR
□ A copy of the certificate of Japanese Language Proficiency Test (JLPT) 2nd grade or above***

④□ A copy of the on-line application form

⑤□ A copy of passport (Photo page only)

***1. Native English speakers do not need to submit any document of language proficiency tests.

2. Please consult Dr. Tsunematsu at case you have difficulties
providing a document specified above.

ONLY APPLICATIONS SENT BY THE OFFICE IN CHARGE OF STUDENT EXCHANGES WILL BE ACCEPTED.

PLEASE MAKE SURE THAT ALL DOCUMENTS ARE SIGNED.

INCOMPLETE APPLICATION DOCUMENTS WILL NOT BE PROCESSED.

PLEASE MAKE PHOTOCOPIES AND/OR SCANNED COPIES OF ALL DOCUMENTS BEFORE SUBMITTING THEM TO YOUR COORDINATOR.
