Dear Sir,

We have been nominated to supply you with the following; we kindly request you to complete this requisition form as a procedure in order to arrange transfer of the supplies to your vessel.

Please confirm the following as an agreement to receive the said supplies;

1. The supply vessel can be moored only along the parallel body of the receiving vessel. The supply vessels ropes are fast safely. On the onset of bad weather, immediate casting off is ready.
2. Whilst we agree to take all possible precautions to avoid contact damages to each vessel, it is understood that ship to ship transfer at Anchorage or at OPL is done at own risk of contact damages to vessels whilst maneuvering and/or during alongside operation where each party indemnify the other against all claims of contact damages arising there from.
3. If stores supplies are to be lifted, necessary lifting gear must be ready and available onboard the receiving vessel. Lifting cranes/derricks on the receiving vessel must be capable of reaching the parallel body area to lift stores from the decks of the supply vessel.
4. Main Fresh waters supply hose is of 3” dia capable of having Y split at the end ; require similar size intake or two intakes with similar combined cross sectional area to receive water efficiently at pumping rate above 60MT.
5. As far as practicable, the main hose itself must be hauled up to insert the hose into the tank opening or insert the reduced elbow of min. dia of 2.5” into Filling/Vent line as the one & only supply line.
6. Any reduction in the size of intakes and pumping height lowers pumping rate resulting in delays. Try to open up an air vent or manhole to insert the supply hose directly into the tank.
7. Sounding / Ullage/ reading of barge tanks and tank tables must be observed before and after pumping. Failure to do so is no reason to dispute after the supply and such is not acceptable.
8. In the event of any discrepancies in the volume, reading on the barge is to be accepted as final.
9. Other stores supplies are provided as delivered to us by the agent’s / suppliers and for their quality quantity and safety, supply vessel operators take no responsibility at the time of ships to ship transfer.
10. Upon the supplies are transferred, the Delivery Receipt is signed in three copies.
11. No remarks in the final receipt are acceptable.
