· Complete the attached application (ink or typed) and submit it to the guidance office no later than April 2, 2012.
· Any additional pages (maximum of 4 total pages) must be single-sided with the applicant’s name on each page.
· Do not submit your application in a report cover. Affix all pages with a paperclip (p. 2 on top) so they can easily be copied.
· Failure to complete ALL sections of this application accurately may disqualify you from scholarship consideration.
· The essay questions (p. 3) are required for scholarship consideration.
· Retain pages 1 and 3 for your records.

This application will be used to award a number of local scholarships. We are looking for responses that make you stand out amongst the other applicants, and reflect your worthiness as a scholarship winner. Your responses to the following essay questions are one of the most important parts of your application.
ESSAY QUESTIONS: Answer the following questions on a separate sheet of paper. Remember to put your name on each sheet and to include the writing prompts with your responses. You may put multiple responses on the same sheet of paper.
