ELECTRICAL HAZARD PREVENTION
Electrical appliances, equipment & tools are grounded, free of damage and splices
Disconnecting switches & circuit breakers properly labeled
Circuit breakers indicate “on” and “off” position
Switches, receptacles & junction boxes provided with covers & plates
Unused openings enclosed with appropriate covers
3’ clearance in front of electrical panels and circuit breakers
Extension cords/power strips are not piggy-backed onto one another
Extension cords have grounding conductors (3 prong)
WALKING AND WORKING SURFACES
Floors, aisles and walkways clear of obstruction
Aisles and walkways visibly marked
Handrail present on stairways with 4 or more steps.
Stairways are clear of obstructions
Floor openings and holes are protected
Overhead storage area guarded with fall protection and a "kick plate" at floor level
PERSONAL PROTECTIVE EQUIPMENT
Employees are wearing required PPE (Eye, hand, foot protection, etc.)
Face Shield available near grinder
Appropriate PPE available when welding is done
PPE is in good useable condition
PPE available in shop and inventory stocked
FUEL ISLAND
Eye protection, nitrile/rubber gloves and fire extinguisher available

GENERAL HOUSEKEEPING
Work areas are free of debris, tools, parts and equipment not in use
Floors and stairways free of liquids and tripping hazards.
Shop layout and appearance is reasonably neat and orderly.
COMPRESSED GASES
Cylinders are legibly marked to identify contents
Cylinders stored with valve caps on, caps closed, and away from heat sources
Cylinders are secured to prevent them from falling or rolling
Oxygen and acetylene cylinders separated by 20 ft. or by a 5ft barrier wall (min)
Empty cylinders marked “empty” with valves closed and caps on
Flashback arresters properly installed on each torch set
VEHICLE MAINTENANCE
At least one non-drive wheel is chocked during vehicle maintenance
Mobile lifts used properly and in good condition
Stationary lifts used properly and in good condition
Jack stands are in good condition; load ratings legible
Lockout/tagout procedures are being followed
Proper ventilation system is provided and used when vehicles are running in the shop
MATERIALS HANDLING/STORAGE
Forklifts are inspected prior to use
Keys are removed from forklift when unattended
LP gas powered forklifts are parked with the tank service valve closed & away from ignition sources
LP gas tanks are stored upright in a designated area away from ignition sources (if stored on side the relief valve on top)
MACHINE GUARDING
All machinery is reasonably clean and properly maintained
Machinery and equipment is securely anchored
Grinders, saws, compressors and similar equipment have appropriate guards properly in place
Workbenches with vises attached are securely anchored
LADDERS
Ladders are in good condition with no broken/bent rungs or visible damage, sharp edges, or splinters
Safety feet are in good condition
Stepladders are less than 20 ft. in length
LOCATION SAFETY
Adequate lighting for vehicle parking area
Location is properly graded and free of potholes and ruts
