This contractual language has been vetted and approved by both Southwest Airlines and Sterling InfoSystems, Inc. Please execute this document by signing off in “CLIENT” section on page 1.
End User Certification:
The End User Certification is a document that details all required action mandated by the FCRA regarding the collection, submittal and receipt of background checks and the information that is held within. By signing this document your company is certifying that the background check will be maintained and used in the appropriate manner.
Please complete authorization on page 2 of this document.
Credentialing Document:
All new clients are required to complete this document as a due diligence procedure. Please complete the following:
Information required:
-Credentialing application (first two pages only)
- The section “General Company Information” on the first page must be completely filled out, but the “Principle of the Company” section only applies to partnerships or sole proprietorships, it is not necessary if it is not applicable.
- The permissible purpose MUST be checked on the top of the second page (people often miss this part)
- A bank reference is required (if you do not want to include your bank account number, that will be acceptable) and at least one trade reference, which can be from anyone with which you do business.
-Screen shot of your website. If you do not have a website, you can supply a utility bill in your company’s name
-A screen shot of your companies listing from the applicable secretary of state website, which can be found here: If you are not listed because of your business type, you can submit a copy of your business license, etc.
-If your company has been in business for less than one year, we will need two of the four following items:
- Copy of utility bill or telephone bill in the business name
- Copy of lease or proof of property ownership
- Copy of bank statement addressed to the end user
- Proof of commercial insurance
