Knowing your family’s medical history can help you with early screening detection and prevention of disease. This information can be shared with your family and health care providers.

2.  Please go back as far as you can in your family tree, and create a diagram on letter or legal size paper that depicts your family’s medical history. You may choose to enter and then print out your family tree using the tools at: http://www.nlm.nih.gov/medlineplus/familyhistory.html

Depicting the following:

·  Major (non-communicable) physical and/or mental illness/addictions

·  Age of onset of major illness (if known)

·  Cause of death/ age of death (if known)

·  Squares = Male; Circles = Female (or other design if shown in “Legend”)

·  Note those who have passed away (i.e. slash, “X,” or other mark)

PART THREE: ANALYSIS OF DIAGRAM

After compiling and organizing the medical data, you will write a two to three page report analyzing and summarizing your findings. This should be a well-written paper, free of spelling and grammatical errors. You should utilize good writing techniques—including logical organization, topic sentences, and transitions.
