1.0WORK EXPERIENCE
1.1Applicant is requested to Use Form 1.1 (attached) to record previous experience. Use one sheet for each contract.
1.2Applicant is requested to list all contracts undertaken in the last 10 years which are of a similar nature and complexity to Package CH0009. In each case, list major quantities constructed.
1.3If the Applicant is a joint venture, the Lead Partner is requested to complete Appendix A1 in its entirety and each of the remaining partners is requested to complete Section 2 of Appendix A1. In addition, each partner of the joint venture is requested to complete Form 1.1 for all contracts undertaken in the last 10 years by the said partnerwhich are similar in nature and complexity to the work that will be performed by that partner.
2.0TECHNICAL SPECIFIC
2.1What is your Company’s experience with underwater rock excavation in proximity to permanent structures? Has your Company ever used a system of bubble curtain or equivalent systems to reduce the blast induced overpressure?
2.2What is your Company’s experience in underwater excavation and embankment construction?
2.3What is your Company’s experience in river diversion in high water velocity (Discharge up to 2500 m3/s)? Provide a list of projects completed and include the location, the year of construction and the total quantity of material placed?
2.4In your Company’s past experiences, what type of equipment have you used to clean the riverbed before the placement of till? Do you have special equipment that can reach and clean the bedrock up to 15 m depth?
2.5Does your Company have experience in the development and exploitation of borrow areas and stockpile areas to produce the quantity and quality of materials required for this project?
2.6Does your Company have experience in the selection and processing of soil and rock materials to produce specific grading requirements?
2.7Does your Company normally perform Jet Grouting or utilize a Subcontractor? If a Subcontractor is used, provide the names of companies you have used in the past and the company you intend to use for this project?
2.8What is your Company’s experience in Jet Grouting installations(Single, Double and triple fluid systems)?
2.9Provide a list of projects your Company is currently involved in where Jet Grouting is being used?
2.10Provide a typical layout and flow sheet of typical Jet Grouting set-up.

2.12What is your Company’s experience in Roller Compacted Concrete dam construction? Provide a list of projects completed and include the location, the year of construction, the total quantity of RCC placed, the average and peak rate of placement per month, the type and capacity of the batch plant, the methods of transporting the RCC to the placement site, the method of placement of RCC on the dam, and the time from start to completion of RCC construction?
2.13Provide a list of projects your Company is currently involved in where RCC is being used?
2.14Has your Company ever been removed from a jobsite for reasons related to RCC work? If yes, please provide details?
2.15Has your Company ever been involved in the construction of bridge piers and abutment, and the launching and removal of a modular bridge? If yes, please provide details?

3.1.1Please provide your preliminary Site Organization that would execute the scope of work of Package CH0009. As a minimum the chart should show the positions for Project Manager, Quality Assurance Manager, Chief Design Engineer, Planning and Scheduling Manager, Material Manager (Including procurement, inspection, expediting and logistics), Site Manager, and the key area superintendents.
3.1.2Please describe equipment required to perform the work at Site; and state how you will source the equipment (i.e. own, hired, purchased)? From what location will the equipment come? How will you mobilize it to Site?
3.1.3Do you have an equipment maintenance program? If so, provide a copy of your most recent fleet maintenance report.
