Medicare Part D Enrollment Information Instructions

CMS requires that all residents/fellows be enrolled in Medicare in order to prescribe Part D drugs effective June 1, 2016. See attached FAQ for details.

Below are links and notes to help with your enrollment process.

1.  Link to enroll online: https://pecos.cms.hhs.gov/pecos/login.do

a.  You may use either your NPPES (NPI) or your PECOS login ID and password to enroll

b.  If you do not prescribe to Medicare patients (e.g.: you see pediatric patients only), you do not need to enroll.

c.  Your current last name must match your name listed on the NPI webpage AND the Social Security Administration, or your application will be returned. Log on to https://nppes.cms.hhs.gov/NPPES/Welcome.do to update your NPI name listing if your name changed recently. This must be done before Medicare Part D application is submitted.

2.  A medical license or temporary training permit is required before you can enroll. ALL Emory GME residents/fellows are either licensed or carry a provisional license/training permit. You must answer “YES” when asked if you have a state license and add your license or training permit number. You may skip certification and DEA sections.

a.  If you already have a full medical license, you should enroll now. If not, wait for your license or temporary training permit approval (typically in mid-June and no later than 7/1 for incoming residents/fellows) before you enroll.

b.  You may check your license or permit approval status on the Georgia Composite Medical Board webpage: https://services.georgia.gov/dch/mebs/startSearch.do. Make sure you select “Resident Physician” under “Provider Type” for training permit approval status.

c.  You may enter your temporary training permit number where license information is requested on the enrollment form. Include 2 zeroes before the 4-digit training permit number.

3.  Make sure you select “YES” when asked if you are registering for the sole purpose of ordering/referring. CMS confirms that “prescribing” is included in this. If you select no, you are taken to a wrong page/form to complete. The correct form you are completing is CMS-885O (attached for your reference).

4.  Select “Licensed intern resident or fellow” under “Ordering and Referring Reason”.

5.  Make sure you select your specialty under “Physician Specialties”, not the one for non-physicians.

6.  You may enter your incoming training program mailing address and contact person for correspondence.

7.  Select “Cahaba GBA LLC” as the Medicare Administrative Contractor (MAC) for Georgia

8.  Make sure you select “Electronic” as signature method.

9.  Your approval status should be provided through an email attachment and can also be verified using your last name/NPI# at the following CMS sites:

b.  Newly approved: https://data.cms.gov/Medicare/Order-and-Referring/qcn7-gc3g

10.  Include your approval notice in the packet to return to GME (if you are incoming), or forward to your program coordinator as soon as it is available so your record in NI is updated. A sample approval confirmation is attached for your reference. This is the document you submit as confirmation of enrollment. An application confirmation is not considered as enrollment confirmation.

a.  If you are previously approved, you may forward either an approval notice copy or a screen shot from the “previously approved” link above as proof.

11.  Even though “validly opt-out” is included in the CMS communications as a possible option, training hospitals have not approved it as an option for Emory GME residents/fellows.

Additional Notes:

In response to problems some resident/fellows run into, below are additional information/clarification to help you avoid issues. Attached are also a few sample pages from the registration site.

  1. Many are asked to mail in a hard copy even though they have completed the online registration. Please do so if asked. Make sure you read your online registration confirmation email to see if you are required to mail in the hardcopy or any other additional documents. If so, hard copies of all supporting documents should be sent to: Cahaba GBA, LLC, Provider Enrollment, P.O. Box 6169, Indianapolis, IN 46206, Tel: 877-567-7271
  2. It is also possible to register using the paper enrollment form CMS-855O as attached. Make sure you fill out the form digitally and completely, print, and sign in BLUE INK before you mail to Cahaba. It is also possible for program coordinators to fill out the form on behalf of the resident/fellow, but the resident/fellow MUST sign in person and in BLUE INK.
