Note:

  1. The Registration & Processing Fees will be waived for accreditation received by the Secretariat during February - March 2016. The amount to be collected during this period is RM50.00 only.
  1. Subsequent application shall be charged according to the following fee structure:

  1. Upon successful accreditation by MAKAF an email stating the accreditation number will be sent to the applicant. An accreditation card will also be sent to the applicant's home address.
  1. If the applicant, at the time of application, is an active licensed referee, active coach, and an athlete currently competing please fill in the achievement columns accordingly.
  1. All forms and fees must be submitted and paid via MAKAF Affiliates ONLY. Forms received from non-MAKAF Affiliates will not be processed, and refund will take at least 3 months.
  1. ALL FORMS received from MAKAF Affiliates must be typed (No hand writing), signed, stamped, strictly in PDF format and via email only. Otherwise the application WILL NOT BE PROCESSED.
  1. All payments of fees shall be paid in full to MAKAF upon issuing of Statement from MAKAF Treasury within 2 weeks from the date of statement.
