Permanent Position Family Physician

Up to 30 hours per week: CommencingApril 1, 2018

Seaway Valley Community Health Centre (SVCHC) is anot-for-profit,community-led organization that provides primary health care, health promotion and illness prevention services focusing on the social determinants of health.

SVCHCis designed to address the issue of equity for citizens facing barriers to accessing the health care system. Wework in aninterprofessional team ofphysicians, nurse practitioners, nurses,dietitian, community outreach workers, health promoters and medical secretaries, using a fully electronic medical record. Additionally, our Team works closely with community health and social service partners from our community and region in order to link clients with the medical and non-medical services they need.

We are looking for primary care providers with the following qualifications:

  • Medical degree and family medicine residency from a recognized university; CCFP designation; licensed to practice in Ontario and in good standing as of April 1, 2018.
  • Interest and experience in working:
  • in community health settings
  • with nurses in an expanded role
  • as part of an interprofessionalteam
  • with disadvantaged groups and marginalized populations.
  • Strong interpersonal and communication skills and sound judgement.
  • Proficiency in the use of computers and relevant software applications; experience with electronic medical records an asset.
  • Experience in program development, implementation, monitoring, and evaluation an asset.
  • Available to work one evening per week.
  • Proficient in English required. French and/or fluency in another language pertinent to our client population would be considered an asset.
