Re: Approval to receive invoices in an electronic form
Pursuant to § 106n of The Goods and Services Tax Act dated March 11, 2004 (consolidated text dated July 29, 2011, Journal of Laws no 177, item 1054, as amended), I hereby
grant the approval
to issue e-invoices by Raben Logistics Polska sp. z o.o. for services rendered to our company.
I accept that the invoices for the services rendered to our company may be issued by Raben Logistics Polska sp. z o.o. as PDF files (in portable document format) and then sent to us
via electronic mail (e-mail).
I hereby designate the following e-mail address to receive e-invoices issued by Raben Logistics Polska sp. z o.o.:

I hereby acknowledge and accept that:
1) The change of the e-mail address designated to receive e-invoices issued by Raben Logistics Polska sp. z o.o. requires a written notification of Raben Logistics Polska sp. z o.o. The change of the e-mail address shall be effective from the next settlement period in which Raben Logistics Polska sp. z o.o. receives the appropriate notification.
2) Failure to inform Raben Logistics Polska sp. z o.o. about the change of the e-mail address shall result in releasing Raben Logistics Polska sp. z o.o. from the liability for ineffective delivery of the issued invoice.
3) This approval does not exclude the right of Raben Logistics Polska sp. z o.o. to issue and send invoices in a paper form.
4) This approval to receive e-invoices may be withdrawn at any time.
In the case of withdrawal of the approval, the next invoices shall be issued
by Raben Logistics Polska sp. z o.o. in a paper form, effective from the day following the day on which Raben Logistics Polska sp. z o.o. receives a statement on withdrawal of the approval to receive electronic invoices.
