ALL Rising Star Awards Support Material is due Friday, April 20, 2018 at 5:00 pm.

Please use this form for all submissions of support material for the Rising Star Awards, the Student Achievement Awards, the Educational Impact Award and the Rising Star “Theatre for Everyone” Inclusion & Access Award. It will help us to better keep track of your submissions and expedite their safe return. Please refer to pages 11-12 of the Rules and Guidelines for important information on Support Material requirements. All schools are REQUIRED to submit support material for review by the Nomination Committee and Evaluators.

MANDATORY: DIGITAL production photos on CD, DVD or USB Flash Drive (DO NOT send prints)

Finished Costumes (photos of costumes during a performance or dress rehearsal are preferred)

EACH SUBMITTED ITEMS MUST BE LABELED WITH THE SCHOOL’S NAME AND THE NAME OF THE SHOW.

**The submission of DVDs comes with certain conditions:

1)  DVDs submitted should be segmented into chapters and must be compatible with Microsoft Windows Operating System or a DVD player.

** Our preference is to receive the full length DVD of the show segmented into chapters by production number. Also acceptable are DVDs that:

1)  contain footage of a dress rehearsal, or

2)  are edited highlights with footage of select production numbers and/or scenes
