This contract is signed by Part A and Part B to conduct the research project "Pollution trend of air particulate matter and its relationship with human health in Tianjin" of the Basic research of healthy harmful effects of air particulate matter on human health(grant No. 2011CB503802) (the Ministry of Science and Technology, People's Republic of China as a 973 project). The two parts should come to an agreement according to relevant provisions based on equal negotiation.

1. Cooperation content
Scientific research fruits achieved by the two parts will contribute to the development in this field of our country.

2. Labor division

Part A: publish papers and patents according to the medical data provided by Part B

Part B: provide the following morbidity and mortality data for Part A from 2008.1.1 to 2011.12.31.

1) Injury morbidity number

Accident time, gender, birth date, address (including counties not in detail), ICD10 coding

2) Cardiovascular morbidity number

high pressure, stroke, coronary heart disease

Diagnose time, gender, birth date, address (including counties not in detail), ICD10 coding

3) Psychosis morbidity number

Diagnose time, gender, birth date, address (including counties not in detail), ICD10 coding

4) Diabetes morbidity number

Diagnose time, gender, birth date, address (including counties not in detail), ICD10 coding

5) liver disease morbidity number

Diagnose time, gender, birth date, address (including counties not in detail), ICD10 coding

6) Cardiovascular mortality number

rheumatic heart disease, stroke, coronary heart disease, myocardial infarction

Death time, gender, birth date, address (including counties not in detail), ICD10 coding

7) respiratory system mortality number

Death time, gender, birth date, address (including counties not in detail), ICD10 coding

8)Psychosis mortality number

Death time, gender, birth date, address (including counties not in detail), ICD10 coding

3. Rights, obligations and duties

Part A: Medical data gained from Part B can only be used in scientific research and should not transferred, sold to the third part.

Part B: Provide real and reliable medical data for Part A.

4. Budget allocation

In three days after the date when Part A received all data from Party B, 3000RMB should be paid to Part B via remittance (deposit bank of Part B: Lanzhou Rd Sub-bank,Industrial and commercial bank of Tianjin). Part B gives invoice for reimbursement to Part A.

5. Achievements

Under the agreement of Part B, Part A publish the data in the form of papers.

6. Protocol commencement, modification and termination

1) operant duration is from the date when this contract is signed to the date when the cooperation contents are finished;

2) When there are irresistible factors such as wars, natural disasters, earthquakes and so on resulting in execution failure of this contract, Part A and Part B bear the losses respectively and don't bear the duties of breaking the contract;

3) If this contract conflicts with national laws, the contents should be altered according to laws;

4) Affairs not mentioned in this contract should be appointed separately based on the principles of mutual benefits and friendly cooperation;

5) Memorandum or enclosure of this contract possesses equal law force as this contract.

7. Others

1) This contract is in triplicate, Part A and Part B hold one piece each other, finance department of Part A holds one piece. They all possess the same law force;

2) Both parts should not reveal the contents of this contract without permission of the opposite side.
