Direct Service Professional

Job Summary:

Direct Service Professionals (DSP’s) provide support to individuals with developmental disabilities by providing training and treatment in the areas of self-care, communication, self-direction, independent living, mobility and learning as directed by individual service plan and state and federal regulation. Adhere to and sustain the mission of the organization by providing the highest quality of services in order to enhance each individual’s skill development, health, freedom, and personal dignity.

Qualifications:

He/she is must possess a High School Diploma or equivalent. Must have completed a minimum of 40 hours in a DSP training program. One year of work experience in a setting providing work, social, recreational or behavioral programs to developmentally disabled (DD) or mentally ill (MI) persons is preferred. A general knowledge of providing basic personal care to residents/patients is necessary.

Responsibilities:

1. Plans, schedules and conducts behavior programs to assist DD and MI residents to attain a

normalized Quality of Life

2.  Monitors and documents DD and MI resident behavior and response to behavior programs

such as: behavior medication, habilitation, rehabilitation, learning ability and social

adjustment, self sufficiency, self care, mobility and independent living.

3. Completes required monthly documentation of behaviors

4.  Under the direction of the QMRP and PRSC monitors resident behavior and conducts episodic programs in response to those behaviors

5.  Provides one-on-ones to meet resident psychosocial needs and goals

6.  Assists QMRP or PRSC in case management of DD or MI residents by assuring Activities of Daily Living (ADL’s) are completed, program quality is maintained, contingency programs are implemented and smoking programs are followed.

7.  Oversees and instructs residents in proper grooming, dressing, room care, table etiquette and other manners

8.  Plan, schedule and conduct community, social and medical/dental outings. Assists resident in purchasing personal supplies and items

9.  Any other duty requested by the Administrator, QMRP or PRSC

QUALIFIED MENTAL RETARDATION PROFESSIONAL

Job Summary:

The Qualified Mental Retardation Professional (QMRP) is responsible for developing, coordinating and monitoring all aspects of treatment for their Developmentally Disable (DD) and/or Mentally Ill (MI) residents. They must approve all patient support plans and interventions, monitor progress of program plans, respond to deficiencies in services provided, provide information regarding changes to patient's family and manage caseload and documentation.

Qualifications:

Holds a Bachelors Degree in a human services field. In addition, he/she has at least one year work experience with developmentally disabled or mentally ill persons.

Responsibilities:

1. Assure that the facility is in compliance with state and federal licensing requirements.

2. Participate in the screening of potential facility residents. Complete all pre-admission and

admission processes.

3. Review client and employee incident reports and take appropriate, timely corrective action.

4. Notify Administrator promptly of all injuries or unusual occurrence and ensure that Regional

Quality Assurance Nurse has been notified

5. Participate in the On-Call Supervisor program.

6. Remain current with trends and advancements in the field of developmental disabilities as well

as changes in pertinent state and federal licensing regulations.

7. Oversee all aspects of implementation of the Individual Service Plan (ISP) for each client

including 30-day, semiannual and annual ISP reviews; IDT meetings and subsequent

documentation distribution.

8. Complete required, daily, monthly, quarterly, semiannual and annual program documentation and

other required reports in a timely manner.

9. Coordinate and assure service implementation with outside services (school and day programs)

through regular monitoring and progress reviews and attendance at outside IEP/ISP meetings.

10.Coordinate and assure service implementation with contracted consultants (Occupational and

Physical Therapy; Speech Pathology and Psychological services).

11. Facilitate client participation in community outings and recreational activities
