SUMMARY:

The family advocate will provide home visiting, transportation, outreach, and comprehensive case management services to families that have been reported to Child Protective Services (CWS) but do not meet the criteria for CWS to open a case. The family advocate must possess an understanding and knowledge of community resources, healthy relationships, parenting skills, child development and the ability collaborate with other community services and programs.

ESSENTIAL FUNCTIONS:

·  Following Client confidentiality and consent procedures, the family advocate will complete an intake assessment to determine client’s history, current situation, and existing support system and to identify client’s strengths and concerns. The family advocate will work with families in developing a family goal assessment case plan that will be structured around the
“Protective Factors” that include:
1.  Parental resilience
2.  Social connections
3.  Knowledge of parenting and child development
4.  Concrete support in times of need
5.  Children’s social and emotional needs
·  The family advocate will provide regular home visits to monitor case plan goals.
·  The family advocate will ensure the collection, maintenance, and confidentiality of all required documentation and data through timely and accurate record keeping.
·  The family advocate will assure high quality, supportive and professional relationships within and outside the agency through considerate, non-judgmental interaction with others.

·  Other duties as assigned.

POSITION SPECIFICATIONS:

• AA preferred in a Human Services related field with 2 years case management experience or equivalent combination of education and experience.

• Personal vehicle and proof of insurance.

• Ability to maintain client confidentiality.

• Communicate effectively orally and in writing.

• Assess strengths and cooperatively develop goals and objectives.

• Work with a minimum of supervision.

• Interact and relate to people in a non-judgmental, culturally sensitive and caring manner.

• Exercise sound judgment in problem solving.

• Organize time and set priorities quickly and efficiently.

• Manage multiple objectives, both scheduled and unscheduled.

• Be diplomatic, tactful and professional.

• Deal with time sensitive materials.

• Knowledge of community services available for client referral
