PLAYER BILL -- INSTRUCTIONS
1.) Please complete the name and address section. Check off which team you will play for.
2.) Players must play all three rounds to be eligible for 54-hole prizes. Purse: tournament record $9,000.
3.) Daily prizes are available. Players are not required to play in all three rounds to be eligible for daily 18-hole prizes.
4.) Register online at , or send a check payable to the GALV by Friday, May 20, 2016 with the $210 fee.
5.) If you are a member of a participating course, please write “member” to the right of the golf course line. Pay the overall tournament fee. Members of host golf courses will be reimbursed for greens fees & cart fees depending on their course membership status and host golf course policies.
6.) Unaffiliated team players: a limited number of spots (5) are available for unaffiliated players who wish to compete. Unaffiliated team players must commit to play all three rounds, and stay unaffiliated throughout the entire competition.
7.) Photocopy the completed form for your records.
8.) Fees for any particular round will be refunded subject to five (5) days withdrawal notice to the GALV (610-437-4982).

2016 LEHIGH VALLEY INTER-CLUB CHAMPIONSHIP FEE:$210 (includes greens fees, carts, lunch all three rounds, prizes (gift certificates), post-tournament buffet).
TOTAL PRIZE PURSE: $9,000(gift certificates, based on a full field of 50 regular players, and 25 senior players.)
OVERALL 54-HOLE PRIZE ELIGIBILITY ($6,000): Prizes ($3,000 per division) awarded to the top-20 regular player and senior player division 54-hole finishers: 1. $500, 2. $325, 3. $300, 4. $275, 5. $250, 6. $225, 7. $200, 8. $175, 9. $150, 10. $125, 11. $100, 12. $75, 13. $60, 14. $50, 15. $40, 16. $40, 17. $30, 18. $30, 19. $25, 20. $25.
DAILY 18-HOLE PRIZES ($3,000 overall, $1,000 per day, $500 per division): awarded daily to the low-5 regular player and senior player division scores: 1. $200, 2. $120, 3. $90, 4. $60, 5. $30.
MAXIMUM OVERALL GIFT CERTIFICATE PRIZES PER PLAYER: $750 – in accordance with USGA rules.
PRIZES TIES: All ties, except for first place (sudden-death playoff), will result in an even-splitting of prizes.

HONORS: Participants in the 2016 Lehigh Valley Inter-Club Championship, are eligible to compete for honors:
1.) 2016 LEHIGH VALLEY INTER-CLUB CHAMPIONSHIP 54-HOLE MEDALIST HONORS (REGULAR & SENIOR PLAYERS):
The regular and senior player individual champions of the 2016 Lehigh Valley Inter-Club Championship qualifies for the 2016 Lehigh Valley Open.
2.) 2016 LEHIGH VALLEY TOURNAMENT OF CHAMPIONS INVITATION: The low twelve (12) regular player stroke averagers (plus ties) and the low six (6) senior player stroke averagers (plus ties) in the 2016 Lehigh Valley Inter-Club Championship will be extended an invitation to the 2016 Lehigh Valley Tournament of Champions. Players must compete in all three (3) scheduled inter-club rounds to be eligible.
3.) 2016 LEHIGH VALLEY PLAYER OF THE YEAR POINTS: The low eight (8) regular player placers will earn 2016 Lehigh Valley Player of the Year Points on a 24-14-12-10-8-6-4-2 basis. Players must compete in all three (3) scheduled inter-club rounds to be eligible.
NOTE: The GALV and its committees, reserves the right to verify handicaps, addresses, and amateur status of tournament applicants as defined by the United States Golf Association (USGA). All applications for entrance to GALV tournaments are subject to acceptance by the tournament committee. The tournament committee reserves the right to limit entries, adjust tournament dates, times, competitive formats, and expenditures, and other tournament procedures to adjust to conditions prevailing at the time of the tournament. Purse amounts are predicated on the estimated number of players in the field that are listed in this tournament application. If less players play than estimated for this tournament, purse amounts will be reduced by a similar or accelerated percentage. All applicants agree to abide by the rules of golf as defined by the USGA, local tournament rules set forth by the GALV and the tournament committee; and any tournament rulings of the GALV. Applicants to this and any GALV tournament hereby release the GALV, its officers, and tournament committee, rules committee, and host course personnel from any and all liability for any event or consequence whatsoever in any way arising out of or relating to the applicant's entry or participation in this tournament; with the sole and singular exception of liability arising out of bad faith or willful misconduct by the tournament committee. Entrance fees are non-refundable; except due to five (5) days advance tournament withdrawal notice to the GALV; or cancellation of play due to severe weather conditions, in which case applicable greens fees will be refunded subject to tournament host golf club's refund policy.

3. click on the "Tournament Sign Up" icon. There is no membership fee.
4. the screen will state: “I do not have an ID and Password”. Hit “Next”.
5. waiver page: Review the waiver. Click on "I agree to the above waiver", click “Next”
6. fill out the membership form, click “Next”, continue with the tournament registration process.
7. register to play in tournaments.
