In consideration of being allowed to participate in any of the programs, activities and events SONOMA INDOOR SOCCER, INC. (the releasee) offers, the undersigned acknowledges and agrees that;
v The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death. While certain rules, equipment, and personal discipline reduce the risk, the risk does exist.
v I knowingly and freely assume all suck risk both known and unknown, even if arising from the negligence of the releasee or others, and I assume full responsibility for my participation.
v I willingly comply with the stated and customary terms and conditions for participation. If, however, I observe significant hazard during my presence of participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.
v I for myself and on behalf of my heirs, assigns personal representative and next of kin, hereby hold harmless SONOMA INDDOR SOCCER, INC., management, their officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, with respect to any and all injuries, disability, death, or loss or damage to person or property, whether arising from negligence of the releasee or otherwise, to the fullest extent of the law
v Player agrees to follow club rules as posted from time to time. Sonoma Indoor Soccer, Inc reserves the right to add/change/cancel hours of operation/field rental times/fee’s/policies without notice.
v By signing below, player authorizes Sonoma Indoor Soccer, Inc to photograph and/or video player for internal/external use.
v Players MUST HAVE SHINGUARDS TO ENTER FIELD. In the event that shin guards are not worn, player will be ejected from field.
v NO GUM, INNAPROPRIATE JEWELRY or CLEATS may enter the field.
v Player acknowledges that Sonoma Indoor Soccer, Inc. is not responsible for loss or theft on the premises or in the vicinity.
