Community Development Project Worker

We are an innovative independent Hospice that supports people facing life limiting illness to live well right up to the end. We believe that everybody should matter and every moment matters, regardless of how ill people are.
We are looking for a Community Development Project Worker to join us on a part time basis (One day a week) for a contract lasting from November 2017 – 30th September, 2018.
In this exciting role, you will have the chance to help us grow our services, increase our volunteering numbers and enable people in the last year of life to live well through self-management and engaging with their local community.
As a Community Development Project Worker, you will help to grow our community links through an asset mapping process. You will also establish new groups of volunteers and test a new intervention to deliver services that support people near the end of life to plan ahead and to connect with their community.
Specifically, you will:

  • Help to plan and deliver our community engagement project
  • Support development of additional asset maps for the locality
  • Inspire local communities to take action to support people in the last year of life
  • Participate in identifying individuals’ support assets and needs
  • Recruit, train, support and enable community volunteers to succeed
  • Act as a link between local communities to share best practice

  • To contribute to the Hospice Community Development programme, working with communities through co-production to support self management and build community resilience in end of life care.

4. BACKGROUND INFORMATION
Strathcarron Hospice is a charity established in 1981 to offer patients diagnosed with a life limiting illness a range of care and professional advisory services. It currently provides high quality palliative and end of life care to a significant and growing number of people living in Forth Valley, Cumbernauld and Kilsyth. It employs a large team of clinical staff, who work across community, in-patient and day services delivering individualised, responsive and holistic support to patients and their families/carers. It also draws on the skills and time of a growing number of volunteers who work in the hospice and in the community to help the hospice achieve its aims.
Over the past few years, the hospice has become increasingly interested in building and supporting help which is provided by volunteers and lay carers to augment care provided by professionals. It recognises the unique contribution of individuals who give time and attention to others in their community who face advancing illness or bereavement. When members of a community are able to support others in the same community, care is culturally specific and sensitive. We believe that with bespoke training, support and supervision, these individuals can make a significant difference to social isolation, practical challenges and gaps in end of life care.
The hospice aims to develop a growing team of skilled volunteers and to work with community leaders and groups to help establish networks of people who will support each other in the face of serious long-term illness or bereavement. We hope that over time these networks will form “compassionate communities”.

5. KEY DUTIES
  • Contribute to the asset mapping process in a defined geographical area.
  • Participate in identifying individuals support assets and needs using circles of support model
  • Assist in the assessment of referrals and matching of befrienders in the communities.
  • Participate in the training, supervision and support of volunteers.
  • Act as an information and signposting resource to volunteers and a range of health and social professionals, third sector and community groups
  • Liaise with community groups, third sector organisations and health and social care professionals to raise awareness and encourage referrals
  • Work with the hospice multi-disciplinary team to raise awareness of the work of the community
  • development team and encourage appropriate referrals
  • Manage and prioritise own work time to ensure the delivery of project outcomes
GENERAL RESPONSIBILITIES
  • Participate in the overall community engagement work of the Hospice.
  • Support the ongoing reporting process for all projects, including data capture and analysis.
  • Maintain patient and staff confidentiality at all times.
  • Understand and adhere to Hospice policy and procedures
  • Identify and address own development needs
  • Promote the philosophy of person-centred support and excellence in end of life care.
  • Act as a representative and ambassador of the Hospice.
Physical skills
  • The physical skills required to undertake this role are minimal however the use of a computer and keyboard is a requirement of the role.
  • A driving license or the ability for independent travel is necessary
Patient/Client Care
  • Support the Community Development Coordinator in her responsibility for the work of Hospice befriending volunteers in the community, ensuring safe and therapeutic relationships
  • Respond sensitively to the individuals and communities that interface with the community development work stream, sign posting internally and externally as appropriate
  • Where appropriate ensure accurate records are maintained in line with data protection, patient confidentiality and other statutory and professional guidance.

Terms and Conditions:The terms and conditions of service are those laid down in line with the Agenda for Change Handbook.

Location: Strathcarron Hospice.

Salary Scale:Band 4, £20,302 to £22,910 per annum, pro rata.

Staff taking up a new appointment with Strathcarron Hospice will normally enter the scale at the minimum of the pay band/range. Any appointment above the lowest point of the pay band/range will be subject to verification of previous NHS/Hospice services or experience outside the NHS/Hospice, which is given in recognition of relevant complete years of experience.

Hours:Part time 7.5 hours per week

Employees may be required to vary the pattern of their working week subject to the exigencies of the service.

Duration:Permanent.

Annual Leave:27 days rising to 29 days after 5 years service and to 33 days after 10 years service plus 8 public holidays (pro rata).

Or

202.5 hours rising to 217.5 hours after 5 years service and to 247.5 hours after 10 years service plus 60 hours public holiday (pro-rata)

Sick Leave & Pay:In accordance with Strathcarron Hospice provisions

Superannuation:This post is superannuable under The Falkirk Council Local Government Superannuation Scheme and your remuneration will be subject to deduction unless you choose to opt out of the scheme.
