TITLE:COMMUNITY HEALTH REPRESENTATIVE (C.H.R.)

PROBATIONARY

PERIOD:SIXTY (60) CALENDAR DAYS FROM INITIAL APPOINTMENT

MAJOR DUTIES:

  • Provides training to the community and teach individual patients abut health and health care.
  • May be required on different occasions to fill-in or assist other CHR’s in their specialty areas.
  • Prepares material for presentation to requesting organizations such as schools, health fair exhibits, parent groups, and may utilize experts in the health field to assist or train in their specialty areas.
  • CHR will maintain certification in First Aide and CPR and will continue to seek training related to general health care.
  • CHR will perform blood pressure reading and other appropriate examinations to check apparently healthy people for the presence of abnormal findings that may require medical attention.
  • CHR will provide requested exams at community gatherings or fairs that include such testing and prepare for referrals for appropriate medical attention.
  • CHR will assist CCHC staff or conduct their own surveys to identify household and individual risk factors for disease and development plans to address findings of surveys.
  • Advocate for patients by making appointments, filing complaints for patients, assisting in filling out forms, assist in interpreting, follow-up care such as therapy, and obtaining additional services.
  • Arrange for transportation by explaining transportation policies, schedules for commercial transportation, and arrangements for patient transportation with clinics/hospitals.
  • Coordinates all patient notifications procedures (letter and phone) for patient screening and appointments.
  • Participates in other CHR program activities as required.
  • Transports patients and clients as determined by the supervisor.
  • Will submit patient reports daily.
  • Will attend all scheduled meetings, in-service training and other training sessions and document training by providing information for files.
  • Other duties as assigned by supervisor.

MINIMUM QUALIFICATION:

  • C.N.A. or PCA Certificate or ability to obtain within 6 months of hire
  • Possess a high school diploma or GED equivalent
  • Must possess a current valid Montana State Driver’s License.
  • Must have a vehicle with proof of insurance

DESIRED QUALIFICATIONS:

  • Associate of Science Degree in Health Related field and/or Certified Nursing (C.N.A.)
  • Indian preference, enrolled member of the Chippewa Cree Tribe preferred.
  • Must possess a CPR card within 3 months of employment.

OTHER SELECTION CRITERIA:

  • Qualified Chippewa Cree members who are honorably discharged veterans will be given preference. DD214 required.
  • Chippewa Cree Tribal Preference.
  • Will be subject to drug testing.
  • If terminated from any positions within the Chippewa Cree Tribe for violations of the Drug and Alcohol Ordinance I-99, must have completed requirements of the ordinance.

KNOWLEDGES, SKILLS, AND ABILITIES (KSA’S):

1. Knowledge in patient education concepts and patient counseling (Please provide examples on how you will accomplish this).

2. Knowledge to communicate, orally and in writing. (such an example is the ability to communicate effectively with others in order to train and motivate, speech classes, presentation to groups).

3. Knowledge of the Rocky Boy Reservation as to its geographic to population groups, health needs, and cultural differences. [Please provide examples on this knowledge]

4. Ability to work both independently and with groups of health professionals. (Please provide examples on how you accomplished this.)
