Supplementary Information and Forms for Service Agreement for Scholarship Grant Under IDEA

Student Responsibilities Prior to Receiving IDEA Assistance

In order to complete the written service agreement, a student who will receive financial assistance from an IDEA personnel preparation grant will first need to meet certain requirements.

1. Student must meet US citizenship or residency requirements listed in the IDEA regulations at 34CFR § 304.22(a).

2. If the student will be receiving financial assistance from another entity while receiving IDEA financial assistance, he/she must determine that there are no conflicts between the obligations of the IDEA financial assistance and the financial assistance provided by the other entity. Such conflicts are not permitted for persons who receive IDEA financial assistance.

3. Student must complete the Certification of Eligibility for Federal Assistance (US Form #80-0016).

4. If the student will be receiving grant assistance under Title IV of the Higher Education Act while also receiving IDEA assistance, he/she must provide documentation of the amount of Title IV grant aid he or she is receiving.

5. Student must enroll in a course of study leading to a degree, license, endorsement or certification related to special education, related services, or early intervention services.

US Citizenship or Residency

In order to be eligible for financial assistance from an IDEA personnel preparation grant, a student must meet one of the conditions listed in 1-3 below:

(1) Be a citizen or national of the United States;

(2) Be a permanent resident of –

· Puerto Rico, the United States Virgin Islands, Guam, American Samoa, or the Commonwealth of the Northern Mariana Islands; or The Republic of the Marshall Islands, the Federated States of Micronesia, or the Republic of Palau (during the period in which these entities are eligible to receive an award under the program); or

(3) Provide evidence from the U.S. Immigration and Naturalization Service that he/she is-

· A lawful permanent resident of the United States; or

· In the United States for other than a temporary purpose with the intention of becoming a citizen or permanent resident.
