This form is to be used to nominate a person for the National Ski Patrol Hall of Fame. This form should be typewritten and include the date the form was prepared. Font size should not be smaller than 9 point. Including this page and the letter, the nomination should not exceed 4 pages.
Section 1-3
Sections are to be filled out accurately. Do not use nicknames.
Section 4
This section is used for the signature of the Sponsor, signatures of at least 3 National Staff Members OR 3 National Board Members OR 3 National Appointment (NA) or Leadership Commendation Appointment (LCA) holders OR 6 patrollers who support the candidate and, on behalf of the National Awards Committee, the signature of the National Awards Advisor. All signatories must be currently registered NSP members. The NA or LCA number must be listed along with the printed name, signature, and the date of signing. All sponsoring and supporting members must sign to indicate their personal support of the nomination and to vouch for the eligibility of the nominee.
Section 5-7
These sections are to be used to indicate the nominee’s NSP membership information. Approximation of dates is acceptable if exact dates are not available.
Section 8
Indicate the nominee’s NSP history. List the nominee’s specific contributions to the NSP at all levels. This should include offices held, committee work, patrol activities, awards, etc. Approximation of dates is acceptable if exact dates are not available.
Section 9
List training courses or workshops such as:

· CPR Instructor or Instructor Trainer
· NSP Instructor Development
