Guideline for Annual Inventory/Asset Checking (Due 30 June 2007)
1. The purpose of the asset/inventory checking is to verify the list of items/equipment purchased by FOE staff and parked at various labs and research centers. The checking involves physical inspection of the items. It is also for the staff in charge or custodian to declare the items under their care and responsibility. Declaration of all equipment under the staff custody is compulsory.
2. The list is to be submitted through the Dean to UAAM (MMU asset monitoring department). UAAM officer may conduct checking to inspect the equipment with the help of asset custodian in FOE.
3. The updated list will also help UAAM update the insurance coverage of equipment under FOE. This is due to the increasing number of equipment being bought in the faculty.
Actions required from the person in charge or custodian of FOE inventory and assets. The following flow is used.
1. Verify and sign (signature, name and date) on the generated list for the items under your custody. Make sure the equipment under your custody is traceable.
2. Update the record if necessary (use blue or red pen).
Update the record using blue or red pen if there is any changes in the record.
For research equipment, information such as account code, project name and sponsor is very useful for identifying the equipment.
Some equipment come together in a set and is needed to work together. For example the probe station may consist of microscope, probe head, probe arm and computer. This information can be written down in the remark section of the record.
2(b)Declare equipment not in the list.
If you are in the custody of an equipment but the equipment record is not in the given list or the declaration has not yet been send to Mr Azwan for asset database key in, please use the attached declaration form to declare the items. The FOE asset database will be updated based on your declaration.
2(c) Submit for disposal recommendation to Lab Management Committee
If faulty equipment is no longer economical to be repaired, you can submit proposal for disposal to the Lab Management Committee secretary Mr Azwan. It will be discussed in the LMC meeting. Please be present in the meeting later. (Please use the forms available at http://foe.mmu.edu.my/lab/staffres.htm. After approval from the Dean has been obtained, please proceed to fill in UAAM asset disposal form at http://intranet/).
2(c) Loss Declaration
If equipment cannot be found, take immediate action to locate the missing equipment. Equipment is considered missing if the person in charge (custodian) cannot find or know the location of the equipment. If all effort to locate the missing equipment before the 24th January 2006 fail, please declare the missing equipment using the UAAM form at Declaration Form For Damages/Stolen/ Lost Equipment/Asset at http://intranet/2005/std_app_form.php (after going through the lab/center supervisor and dean).
Please give a copy to Mr Azwan (Asset Officer)
2(d) Equipment Transfer
If the equipment has been transferred to another person but not yet updated in the record, please request the recipient of the equipment to sign beside the equipment (Include recipient name and date of transfer with the remark Transfer To).
Future inter lab or center transfer will require transfer form to be filled in together with the asset list (generated from FOE asset database).
3. Submit to lab/center staff
Submit to lab/center staff if the equipment is parked in the lab or research center staff if the equipment is parked in the research center. Refer to table 2.
4. Lab/center staff will pass the verified asset list to lab/center supervisor for verification and signature.
5. Lab/center staff will submit the completed asset list to Mr Azwan (Alcatel Lab) to be compiled
