Guidelines for formatting Assessments

June 2010

Please format assessments according to the following criteria:

1. Word documents

2. Use Times New Roman font-12pt

3. Organize as unit assessments with separate answer key for each. Do not include space on the test for student answers. Students will use a separate answer document.

4. Compile unit assessments to create EOC (end of course) assessment with answer key. Do not include space on the test for student answers. Students will use a separate answer document.

5. Sample format for each answer key:

(If a problem includes a graphic, add a line below the question number to cite the source of the graphic or graphics used.)

6. Create a separate sheet for student-recorded answers for each unit or EOC (end of course) assessment. Students will not record answers on the assessment document. They will record answers on this answer sheet.

7. Do not embed correct answer or HSCE in the assessment document.

8. Place a header and footer on each page of each assessment.

Header centered:Wexford Missaukee Intermediate School District

Name of Course (ex. Social Studies grade 4)

Unit (ex. Unit 4 Exploring Economics)

Footer right aligned:version of the document “Arial” font-8pt

9. Number pages – centered

10. Use page breaks.

11. Super/sub scripts – make these larger for ease of reading

12. Each assessment will need directions. Place directions on the first page before typing item number 1.

For example

Directions: Identify the letter of the choice that best completes the statement or answers the question.

12. Single space stem

Double space options/foils

Lower case letters for option/foilchoices

Center any graphic or reference below the stem. If there is a graphic or reference be sure the stem says, “Given the chart below….” or similar appropriate reference.

Sample:

  1. This is a sample stem that is longer than one single line. Be sure to indent the second line, causing the question number to stand out from the stem. You might type, “Refer to the picture below to answer the question.”

a. This is the first option/foil.

b. This is the second option/foil.

c. This is the third option/foil.

d. This is the fourth option/foil.

13. Any graphic that you draw, any graphic that also includes a text box, and graphic that has multiple parts must be positioned in front of text. Move the cursor ahead the necessary number of lines to make room for the graphic.

14. Complete one Social Studies unit and the accompanying answer key/rubric.In World Languages please complete the assessment components for novice mid with accompanying answer key/rubric. Please save on the desktop and upload to the website.

15. After all the items foreach assessmentare entered, check to see that there are approximately the same number of a's, 'b's, 'c's and 'd's as correct responses. Readjust the options if necessary (without disrupting the order ofnumeric responses, i.e. increasing or decreasing order).
