As a part-time faculty member, you will be a vital and integral part of our academic programs. As a professional educator, you understand the need for submitting the required employment credentials.

To ensure that Spalding University is in full compliance with our accreditation organization, the Southern Association of Colleges and Schools Commission on Colleges (SACSCOC), please make certain that you have submitted the following list of documents to the Administrative Coordinator for HR.

You will be assigned a SU e-mail address after submission of the paperwork and you are requested to utilize it for all university business. If you have previously worked for Spalding in a teaching capacity within the last five years, please contact our office to make sure that your credential file contains your most current information. Your salary will be payableonly afterthe completion of all required credential documentation in the Office of the Provost and you have signed a SU contract.

_____An official transcript from each college and/or university previously attended. Official transcripts are those sent directly to Spalding from the issuing institution. Any transcript marked "issued to student" or any transcript that is hand delivered is not considered an official transcript. Have transcripts sent to the Administrative Coordinator for Human Resources, Spalding University, 845 S. Third St., Louisville KY 40203 or proof that the transcripts have been requested. Please contact the Administrative Coordinator if the institution provides an electronic transcript.

_____Federal I-9 form

Attach a copy of document(s) from the list of acceptable documents on back of the I-9

_____Federal tax form, W-4

_____ State tax form

_____TWO current letters of professional reference

_____Confidentiality Statement (signed)

_____University Policy Manual Statement (signed)

_____Payroll form for direct deposit (required)

Attach a voided check from current checking accountto initiate the required direct deposit of

your payroll check.

A Spalding ID is issued for library use, security purposes and a bookstore discount. Please arrange a time for this short process with the Administrative Coordinator.
