PARENT/GUARDIAN FIELD TRIP CONSENT FORM

The purpose of this letter is to inform you about a proposed extra-curricular field trip involving your child, and to seek your support and written permission for your child to participate. Please note that participation in this activity is strictly voluntary. The cost of the trip reflects your child’s expenses only. No student will be denied access to a field trip or activity due to an inability to pay. Please contact (the school principal) to discuss a fee waiver.

Date of Field Trip: Wednesday November 10th

Location of the Field Trip: Grouse Mountain, North Vancouver

Nature of the Field Trip: Coquitlam Open Learning – Leadership opportunities for the COL Immersion students

Departure Time: 8:15am Anticipated Return Time: 3:30pm

*Please make sure your child is dressed appropriately for the weather since some of this field trip will take place outdoors.

Itinerary:

8:30 -- Arrive at Coquitlam Open Learning (Mundy Rd.)
9:30 -- Arrive at the Base of Grouse Mountain
9:45 to 10:15 -- Skyride to Alpine Station
10:30 to 11:15 -- Curriculum Section (K-4 will do the “Bears of North America Tour” during
which they will hopefully see the live Grizzly Bears in their habitat; Grade 5-9 will
participate in the “Risky Business” workshop, which is about endangered species)
11:30 to 12:15 -- Interpretive Wildlife Habitat Session or Ice Skating
(depends on availability of snow)
12:30 to 1:00 -- Lunch Break. Bring your own bag lunch.
1:15 to 2:00 -- hiwus First Nations Cultural Presentation
2:15 – Skyride to Base
2:30 – Depart Grouse Mountain
3:30 – Arrival back at COL (Mundy Rd.)

Cost:

Cost to the student is $10.00.

Transportation:

Bus transportation will be provided by Coquitlam Open Learning

While schools staff will take reasonable steps to prevent injuries to students, some degree of risk is inherent in the nature of activities or during transportation, and may occur without fault on the part of the student, school board, its employees or agents, or the facility where the activity is taking place. By allowing your child to participate in this activity, you are agreeing that the activity described above is suitable for your child, and that there is a risk of injury associated with the activity and travel.

My child and I understand that both the district and school’s code of Conduct applies during the field trip.

I will be responsible for any costs caused by my child’s failure to abide by the Codes of Conduct, including any costs to send my child home.

Cost of Extra-Curricular Field Trip: $10
