School Selection Worksheet

Instructions & Notes:

First, carefully review the list of organizations participating in CAP this year. Contact the Connelly Foundation or the organizations directly with any questions you have.
Indicate your preferred organizations and back-up choices for two field trips for each grade(grades 4-8). If you are combining any grades for field trips to make the best use of your bus capacity, simply write the same organizations down for those grades. (Remember to account for chaperones in planning your bus capacity.The cost of any additional buses you require above your allotment will be the responsibility of the school.)
Please indicate your preferred organizations for two in-school assemblies that will come to your school.
If your school is eligible for a residency this year, please provide your first, second, and third choice of program.

Return this form to the Connelly Foundation by email (), fax (610-834-0866), or mail by Monday, May22nd. Receipt of all forms will be acknowledged. Please contact me if I fail to do so within 2 business days.

Once we have received each school’s preferences, the Connelly Foundation will determine the optimal allotment of events and notify each school in September of the organizations you will seein 2017-18. At that point, it is the school’s responsibility to contact the organization to arrange for specific dates and times for each event, and to reserve buses.Please note we understand you will only have estimated numbers of students at this time and recognize the actuals scheduled may differ slightly than those provided here.
