  • Veterinary Letter: completed by veterinarian with committed rates, one letter per participating clinic. Template provided on the website.
  • Evidence of Matching Funds: Must demonstrate that the committed amount is reserved specifically for this project.
  • Letters of support from Partners: If the project includes a partnership, a letter of support must be provided from the partner.
