For determination of eligibility please return this form with a COMPLETED and SIGNED copy of your parent’s /guardian’s 2017 Form 1040. This information is required to verify your eligibility in compliance with the U.S. Department of Education Regulations and must be submitted by all applicants whether or not they are applying for financial assistance. If your parent’s/guardian’s 2017 household federal income tax return has not been completed, a copy of your parent’s /guardian’s 2016 Form 1040 can be submitted to AAP for decision purposes only. A copy of the 2017 Form 1040 is required and must be submitted to AAP no later than May 15, 2018 to confirm your STP participation.
