The West Monona School Foundation is very excited to be offering five $1,000 scholarships for West Monona seniors who are graduating in 2015. This will be the second year that the West Monona School Foundation has been able to offer scholarships. It is the intent of the Foundation Board to continue to offer scholarships every year long into the future.

You are invited to apply for one of the West Monona School Foundation Scholarships. The recipients will be announced at the annual senior awards program. Scholarship money will be sent to the school of your choice in January when you have enrolled and are attending second semester classes.

All West Monona graduating seniors are eligible to apply for these scholarships if you plan to attend any training or school beyond high school. This is not limited to two or four year colleges. Any formal training program will qualify you for the scholarship.

Applications must be completed and turned into the high school counselor office by April 15, 2015 to be considered. An editable copy of the form may be found on the West Monona School Foundation web site at www.wmschoolfoundation.org . You may also attach additional pages to this printed application to submit it before the deadline.

As you complete this application, please take special care to use what you have learned to make it a high quality application. The quality of your writing, your sentence structure, your grammar, your spelling, and your content will also be taken into account when your application is evaluated.

III. Personal Resume:

Write about yourself, your home, family, and interests, and your future educational or vocational plans in a way which will give the committee the best picture of you as an individual.

IV. Need for Financial Assistance:

Indicate how you plan to finance your post-secondary education. Relate any special circumstances which have a direct bearing on your need for financial assistance.

V. School Activities:

List all high school activities in which you have participated. After each activity indicate the grade(s) in which you participated by using the symbols 9, 10, 11, 12. Then list any leadership positions you may have held in this activity and indicate the grade when the position was held. Also describe any special circumstances which may have limited your participation in school activities.

VI. Community Service:

Describe the ways in which you, as a volunteer, have been of service to your community in general or to certain people or organizations in your community in particular. This service may have been performed as a member of an organization or on your own as an individual. Indicate any leadership roles you may have had in this area. Describe any special circumstances which may have limited your participation in community service.

VII. Evaluation by School Official (Superintendent, Principal, Counselor, or Teacher):

The evaluation should provide information about the Student on the following areas and should be on official school stationery.

A. Personal attributes

B. Abilities, talents, and interests

C. Scholarship, study habits, and probable success in college or vocational school

D. Any other pertinent or unusual factors which should be considered by the selection committee.

E. Be certain that the school official’s title and signature are included on the letter
