Important enrollment information: Your actionrequired

Under the new labor contracts,effective January 1, 2017, yourcurrent Medicare medical plan option sponsored by Verizon will be converted to a national Medicare Advantage PPO plan.

In order to be enrolled in this option, you must be enrolled in Medicare Part B. Our records indicate you are notcurrently enrolled in Medicare Part B. You have an Initial Enrollment Period in which you can enroll in Medicare Part B.

What you need to do

1)Enroll in Medicare Part B coverage now

  • Go to your local Social Security office.
  • To ensure that your Medicare Part B benefits are effective as soon as possible, it is very important that you apply for your Medicare Part B benefits.

Verizon has partnered with SSDC Services to answer any questions or concerns regarding your Medicare Part B enrollment.Please contact SSDC Services at 866.729.9727 ext. 390 between 9:00am and 6:00pm, Eastern time, Monday through Friday, to speak with a service representative.

2)Once you are enrolled in Medicare Part B

Once your local Social Security office has processed your enrollment, you will receive a letter describing when your Medicare Part B benefits will take effect and how much your monthly premiums will be. For most people, Medicare Part B premiums are deducted from their monthly Social Security benefit.

You will also be provided with a HealthInsuranceClaimNumber(HICN).Thisis thenumber that theSocialSecurityAdministrationassigns you when you enroll in Medicare.You can finditon yourMedicarePartBID cardas well as your health insurance claims and otherpaperwork.

Call the Verizon Benefits Center at855.489.2367 nolaterthanNovember 18,2016toprovideus with your Medicare Part B effective date and HICN.RepresentativesareavailableMondaythrough Friday from9amto5pm, Easterntime. If you do not call to provide theinformationby this date,enrollmentin the Verizon-sponsored national Medicare Advantage PPO plan maybedelayed.

3)If youdo not want to be enrolled in the national Medicare Advantage PPO plan

To disenroll from coverage under the national Medicare Advantage PPO plan or to see what other medical plan options are available to you, please visit BenefitsConnection at verizon.com/BenefitsConnection or contact the Verizon Benefits Center at 855.489.2367during the upcoming Annual Enrollment period.

Please note: If you opt out of Verizon medical coverage, you will also lose your prescription drug coverage with Verizon.

You will receive additional communications over the coming weeks regarding the new national Medicare Advantage PPO plan and the upcoming Annual Enrollment period.
