Dear Parents/Guardians of Team Inspire Students:

The seventh grade class trip to Medieval Times has been scheduled for Thursday, May 4th. Please review the following information.

ITINERARY

WHEN:Thursday, May 4th, 2017

School buses depart from MTMS at 9:30 a.m.

Return to MTMS 2:00 p.m.

Students will follow their normal bus schedule to and from school.

WHERE:Medieval Times, Lyndhurst, NJ

ACTIVITY:Students will participate in the Medieval Times show. This trip is

part of the Social Studies Middle Ages interdisciplinary unit. The Medieval Times dinner and tournament is a live horse show in an enclosed arena. Persons with respiratory conditions, allergies, or asthma should attend at their own risk. Medieval Times uses stroboscopic and pyrotechnic effects.

COST:$57.00 (includes transportation, lunch, cheering banner, and activities)

LUNCH:Lunch is: garlic bread, oven-roasted chicken, sweet corn cobette, herb roasted potato, beverage, and a chocolate chip cookie.

*Vegetarian and kosher platters are also available.

Please make note of your preference on your child’s permission slip (page 3).

PERMISSION SLIPS:

COMPLETEDpermission slips MUST be returned by Friday, March 24th,with a $57.00 check or money order made out to: Monroe Township Middle School in an envelope with your child’s name and homeroom teacher’s name on the front of the envelope.

In order for students to attend the trip, it is necessary for parents to complete the permission slip (page 3) in its entirety. COMPLETED PERMISSION SLIP AND CHECK WILL NOT BE ACCEPTED UNLESS PLACED IN AN ENVELOPE WITH YOUR CHILD’S NAME AND HOMEROOM TEACHER ON IT.

Return page 3 to your child’s homeroom teacher by Friday, March24th, and keep the information copy. There will be NO REFUNDS after April5th. We need to call Medieval Times with the number of tickets needed on April5th.

GROUPS:Groups will be selected by the students at a later date.

WHAT TO WEAR:Although this in an indoor activity, the arena is large and might be cool.

We recommend that students dress in layers such as long pants, a shirt,

a sweatshirt or a sweater, and a coat. Appropriate clothing is expected.

WHAT TO BRING:Students are allowed to bring purses or wallets, money and a camera. The camera may only be used in accordance with Medieval Times rules and regulations. STUDENTS ARE RESPONSIBLE FOR THEIR BELONGINGS AT ALL TIMES. THE SCHOOL DOES NOT TAKE RESPONSIBILITY FOR BROKEN, LOST, OR MISSING ITEMS.

DO NOT BRING:DO NOT BRING GUM, FOOD, DRINKS, BOOK BAGS, OR OTHER ITEMS

NOT ALLOWED IN SCHOOL, BUS, OR FACILITY. ANY ELECTRONIC DEVICES SEEN WILL BE CONFISCATED AND RETURNED TO A PARENT AT SCHOOL. PLEASE REFER TO THE MTMS CODE OF CONDUCT. We cannot leave anything on the bus. Students must limit what they bring, including but not limited to, a purse/wallet, camera, and jacket.

SOUVENIRS:Students might have time before the show to purchase souvenirs. Vendors will stroll around during the performance selling the more popular souvenirs.

For the safety of all students, some souvenirs are to be kept in their bags at all times and are not to be played with on the bus. Anyone found playing with these items (swords, etc.) on the bus will have their souvenirs confiscated.

FIELD TRIP PERMISSION SLIP

designated deadline of March 24th, 2017.

Medications Being Taken:

If your child has a life threatening medical condition, (i.e. asthma, anaphylactic reaction) which requires medication, please contact the school nurse for a request for self-administration of medication form which your doctor must complete and sign. The completed form must be returned to the school nurse prior to the date of the field trip. Medication must be in the original prescription labeled container. For any questions regarding medication on field trips, please contact the school nurse at 732-521-6042 and press “2” to reach the nurse’s office.

I have read the contents of this form and hereby give permission for my son/daughter to attend the field trip. I also understand that school officials have the right to conduct reasonable searches of student backpacks or other belongings. As per Board of Education Substance Abuse Policy #5330, and in accordance with N.J.S.A.18A:40A-12, if I cannot be reached, I give the district my permission to seek emergency medical treatment for my child and be tested for controlled dangerous substance if there is reasonable suspicion reported.

Chaperone Volunteer Form

  1. We will notify you by April 7, 2017 if you have been randomly selected to chaperone.
  2. If you do NOT hear from us, you have NOT been selected. We generally need only 4 to 6 parent chaperones per team and about 4 times that many volunteer!
  3. If you are selected, we will have a brief meeting with our chaperones the morning of the

trip, in our PAC, to discuss responsibilities.
