PETITION FOR FINDING
A CHILD IN NEED OF SUPERVISION

The Navajo Nation through the undersigned counsel petitions this Court for finding a child in need of supervision of the above named child for the offense of [CHARGE DESCRIPTION] and in support of this alleges the following:

1.  This Court has jurisdiction of the subject matter and parties of these proceedings pursuant to 9 N.N.C. § 1055, subparagraph (a) and (d), Navajo Nation Children’s Code.

2.  The above named minor child is [#] years old, is believed to be an enrolled member with the Navajo Nation, C# [000000] and resides at: [PHYSICAL ADDRESS].

3.  The minor child’s parent(s) is/are [NAME OF PARENT and NAME OF OTHER PARENT], who is/are believed to be enrolled member(s) of the Navajo Nation and residing at the same residence as minor in paragraph two.

4.  That said child is a CHILD IN NEED OF SUPERVISION, pursuant to 9 NNC §1002 (G), (3), because the child has committed an offense not classified as criminal or one applicable only to children, [CHARGE CODE], the offense of [CHARGE DESCRIPTION].
