CLASS TITLE:HEALTH FACILITY EVALUATOR II
ALLOCATION FACTOR(S)
This is professional work coordinating on-site surveys of health care facilities and programs for compliance with federal and state licensure and certification requirements. Serves as team leader performing survey functions in facilities providing direct care to clients and residents. Provides guidance to the assigned team members regarding more complex issues and assists in training surveyors.

- Serves as team leader during on-site inspections of health care facilities.
- Participates in conducting on-site inspections of health care facilities.
- Approves corrective actions taken by health care facilities to rectify deficiencies.
- Responds to inquiries from health care facilities concerning deficiencies and code requirements.
- Conducts complaint investigations and reports on findings, making appropriate recommendations.
- Plans and organizes survey teams for scheduled surveys.
- Reviews fiscal, medical, and personnel documentation for completeness, accuracy, and quality of client services.
- Collects survey data and transfers data to reports for submission to proper governmental personnel.
- Conducts entrance and exit interviews with service providers.
- Prepares reports and correspondence.
- Trains employees in methods of health care facility surveying.
- Performs related work as required.

- Knowledge of the principles and practices of health care management.
- Knowledge of health care facilities and operations.
- Skill in the use of computer equipment and software to enter, retrieve, and manipulate data required to perform licensure and certification functions.
- Ability to coordinate work assignments.
- Ability to interpret rules, regulations, laws, and guidelines relating to health care administration.
- Ability to evaluate data and make recommendations.
- Ability to establish and maintain effective working relationships with others.
- Ability to communicate effectively.
- Ability to prepare reports.
MINIMUM QUALIFICATIONS
- A bachelor's degree from an accredited college or university and three years of professional experience in health, social, or rehabilitative services, health care or social services administration or planning.
- A master's degree from an accredited college or university can substitute for one year of the required experience.
- Professional or nonprofessional experience as described above can substitute on a year-for-year basis for the required college education.
