PERMISSION FOR TRANSPORTATION: The YMCA Staff has my permission to transport my child to and from program location in the event of an emergency. I understand reasonable precautions will be taken to ensure the safety and supervision of my child.
MEDICAL WAIVER: In the event that my child requires emergency medical treatment and I cannot be reached, I hereby authorize the YMCA Staff to make arrangements to transport my child to the nearest hospital/emergency medical facility. I give my consent for any and all necessary medical treatment, if, in fact my child requires the attention of a physician.
WAVIER: I understand that YMCA activities have inherent risks and I hereby assume all risks and hazards incident to my participation in all YMCA activities including transportation to and from said activities. I further waive, release, absolve, indemnify, and agree to hold harmless the YMCA, the organizers, volunteers, supervisors, officers, directors, participants, coaches, referees as well as persons or parents transporting participants to or from such activities from any claims or injury sustained during my use of YMCA facilities or participation in any YMCA activity whether located on YMCA property or not.
PHOTO RELEASE: I give my consent for pictures taken of my child involved in YMCA programs to be used for future YMCA promotions or display.
REFUND/TRANSFER POLICY: I understand that the YMCA has no refund policy. Details of the policy are available at the Member Services Desk.

Withdrawal/Refunds/Transfers:Please inform the Youth Sports office’s () if you are withdrawing your child from a sport for any reason, please note that there is $15.00 Processing Fee that will be charged for any withdrawal. The option to receive a refund or credit is at the discretion of the Youth Sports Director. Uniforms are non-refundable… After uniforms are ordered, there will be an additional $40.00 fee to switch a team.
Financial Assistance:Financial Assistance is available. Applications must be received by October 12thand are subject to age group/sport availability.
Practice/ Game Structure (50/50):50/50 Format will consist of 30 minute practice followed by a 30 minute game on Saturdays. The season will consist of 8 games. Game times will vary. Some indoor soccer games may be played in Chasco.
Capacity Numbers:Registration may end beforeNovember 7th due to reaching maximum registration numbers. Please register early. We are forming teams as registrations are being taken. The earlier you register the better chance you have of getting your request.

Equipment and Uniforms:All Players are required to purchase the Game Plan at the YMCA. Uniforms must be worn at games
Or the player will not be able to play. Shin guards are recommended but not required. NO CLEATS!

Pictures:Picture Day - TBD. Teams and individual pictures will be scheduled. Pictures
will be done by ATXPhoto. Picture information and order forms will be on line at (hutto.ygametime.com). You will need to order and pay for pictures on picture day. Pictures will be directly mailed to you from ATX Photo
Coaching:Head Coaches and Co-Head Coaches must fill out a volunteer application and provide a copy of your driver’s license.Coach Clinics will be held prior to every season for each sport.Coaches must complete a volunteer application every year.
YMCA Philosophy:The YMCA is an instructional league, not competitive. The main focus is placed on the children’s skill development and on sportsmanship; therefore, the score will only be kept in older age groups. We build strong kids, strong families and strong communities.
